The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Starboard side main deck portable extinguisher is overdue for required servicing. 46CFR176.810(b)(1)(iv)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired. Life float retro has been painted over and several areas need to repaired IAW manufactures recommendations and products. 46CFR176.808(b)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Operational test of the means provided for pumping bilges. Vessel is unable to operational test the installed bilge system. 46CFR176.804

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. A space with a through-hull fitting below the deepest load waterline, such as a lazarette; a machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and a space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. All of the vessels high level alarms are inop. 46CFR182.530(a)(1),(2),and (3)

The float-free link required by § 180.130(a)(1) must be: Certified to meet § 160.073 in subchapter Q of this chapter, or other standard specified by the Commandant; Of proper strength for the size of the life float or buoyant apparatus as indicated on its identification tag. Float-free link does not have an identification tag. 46CFR180.137(b)(1), and (2)

The equipment required for lifefloats and buoyant apparatus must meet the following specifications:The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. Waterlight lanyard is less than 18 feet in length. 46CFR180.175(f)(4)

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck. Reach rod and screw-down valve are inoperable. 46CFR182.510(d)

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Engineroom has excessive oil in the bilge. 46CFR185.100

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:Charts of large enough scale to make safe navigation possible;U.S. Coast Pilot or similar publication;Coast Guard Light List;Tide tables; and Current tables. Vessel still needs to prove up-to-date charts are onboard the vessel. 46CFR184.420(a)(1), (2), (3), (4), and (5)

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following:The location of emergency exits, survival craft embarkation areas, and ring life buoys;The stowage location(s) of life jackets;Either:(i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, or (ii) That passengers may contact a crew member for a demonstration as appropriate, prior to beginning an oceans or coastwise voyage;The location of the instruction placards for life jackets and other lifesaving devices;That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master; and If the vessel is operating with reduced manning or equipment requirements in § 176.114 of this chapter. Passenger Safety Orientation has not been established. 46CFR185.506(a)(1),(2),(3),(4),(5), and (6)

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. Chemical testing program is not established. 46CFR16.201(a)

The master of a vessel shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. The passenger count shall be available to the Coast Guard upon request. Vessel does not have a passenger count system established. 46CFR185.504

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of heavy weather. The emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in § 185.512. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel does not have heavy weather emergency instructions posted. 46CFR185.510(a)&(b)

Stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel does not have a station license. 47CFR80.13(a)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator. Vessel master does not have a marine radio operator permit. 47CFR80.159(e)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel does not have a FCC Safety Radiotelephony Certificate. 47CFR80.903

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. The vessel has two different names, old name and new name marked on the bow. 46CFR67.123(a)

The name of the vessel must be marked or painted in clearly legible letters on each life float paddle. Life float paddles are missing the vessel name. 46CFR185.604(g)

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Container missing required marking. 46CFR185.614

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. Cabin emergency escape not properly labeled. 46CFR185.606

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit is expired. 46CFR184.710

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Starboard forward cabin life jacket stowage identification is unreadable. 46CFR185.604(f)

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Emergency broadcast placard is missing. 46CFR184.506

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Engineroom portside vent closure is frozen open, and the starboard one has a broken hinge. 46CFR176.800(a)

The master of the vessel shall ensure that each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel is not conducting monthly EPRIB testing. 46CFR185.728(a) & (c)

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part. Crew training is not being conducted. 46CFR185.420(a)

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Abandon ship and man overboard drills and training are not being conducted. 46CFR185.520(a), (b), (c), and (f)

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Fire drills and training are not being conducted. 46CFR185.524(a) through (d)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel's anniversary date is October 13th. Vessel did not complete annual inspection within 3 months of the anniversary date. 46 CFR 176.500(b)(1)

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Vessel's engines do not shut down upon actuation.

Exhaust gas must not leak from the piping or any connections. Vessel's engine exhaust is leaking into the engine space.

Each item of lifesaving equipment not to be in serviceable condition must be repaired or replaced. Vessel's ring life buoy is completely filled with water.

A vessel not required to have a power driven fire pump must have at least three 2 1/2 gal buckets with an attached lanyard with the words "FIRE BUCKET" stenciled in contrasting color on each bucket. Vessel doesn't have a working fire pump nor fire buckets.

Vessels operated solely within 20 nm of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of 80.1101(c)(2). Vessel missing MMSI number from VHF radio. Vessel restricted to 1000 feet from shore until rectified.
