Each vessel constructed of wood certificated to operate on a limited coastwise route in cold water must be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate at least 67% of the total number of persons permitted on board; current IBA has not been serviced. Vessel limited to 3 miles from shore till 01 OCT 2024 when warm water season ends. As of 01 OCT 2024, vessel limited to 1 mile from shore.

On a vessel of at least 26' in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. High water alarm panel is currently inoperable and needs to be repaired.

A vessel of more than 65' must carry a minimum of 3 life buoys of not less than 24" in diameter. At least one life buoy must carry one floating waterlight, unless limited to daytime operations. Vessel's waterlight is inoperable. Repair or replace waterlight.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Gasket in the watertight door in the engine space is misaligned. Re-align or replace gasket to make space watertight.

On a vessel of more than 65' in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Speaker on aft deck is currently being painted and needs to be re-installed and tested.

On a vessel of more than 65' in length, the minimum capacity of the fire pump must be 50 gpm at a pressure of not less than 60 psi at the pump outlet. The pump outlet must be fitted with a pressure gauge.

A vessel must carry adequate and up-to-date: charts, U.S. Coast Pilot, CG Light List, tide tables and current tables. Vessel needs to provide up to date pubs and charts.

Each vessel constructed of wood certificated to operate on a limited coastwise route in cold water must be provided with inflatable buoyant apparatus (IBA) that will accommodate 67% of the total number of persons permitted on board. IBA is currently being serviced and not on board the vessel. Vessel limited to three miles of land in warm water months (July 1 - September 30) and within 1 mile of shore during cold water months (October 1 - June 30) till IBA has been properly serviced and placed on board.

A vessel must have one or more ring life buoys as follows: (3) A vessel of more than 19.8 meters (65 feet) in length must carry a minimum of three life buoys of not less than 610 millimeters (24 inches) in diameter. Provide two serviceable life buoy.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Ensure above engine room watertight door that the watertight bulkhead is watertight.

Rules in this subpart (Rules 20-31) (§§ 83.20 through 83.31) shall be complied with in all weathers. Ensure Starboard navigation light is operational.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Ensure first-aid kit is stowed in a watertight container.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Determine cause of port shaft noise at 1200-1400RPM.

Penetrations of required watertight decks and bulkheads by any rigid plastic pipe are prohibited unless the following requirements are met:One or more metallic shutoff valves must be installed adjacent to the fitting in one of the following ways: (A) Only one metallic shutoff valve must be installed if it is operable from above the bulkhead deck; (B) If two metallic shutoff valves are installed, one on either side of the bulkhead, they need not be operable from above the bulkhead deck provided immediate access to both is possible; Plastic pipe penatrations centerline, port and starboard side of forward engine room bulkhead shall have metallic shut-off valves. By next Drydock.

An adult life jacket must be provided for each person carried onboard the vessel. COI requires 65 adult life jackets, there are only 56 onboard. Vessel is restricted to 53 passengers and 3 crew until 9 more placed onboard.

Each vessel on a limited coastwise route in cold water must be provided with an inflatable buoyant apparatus to accommodate 67% of persons onboard. IBA being serviced. Vessel is restricted to operations within 3NM of shore until the IBA is back onboard or until 30 Sept 2021.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. All employees have not completed pre-employment drug testing

A marine employer whose vessel is involved in a casualty shall be able to conduct alcohol testing within 2 hours. Vessel does not have enough alcohol swabs onboard.

Each vessel that operates beyond 3NM from shore must have a Category 1, 406 MHz EPIRB installed. EPIRB does not have a NOAA registration associated with it.

A vessel must carry one floating waterlight unless it is limited to daytime operations. Vessel waterlight battery is expired. Vessel is restricted to daylight operations.

Prior to getting underway on a voyage or as soon as possible thereafter, the master shall make a suitable announcement to the passengers. Safety orientation sheet does not have an accurate location for life jackets.

A vessel more than 65ft must have a fixed installation PA system audible throughout accommodation spaces. PA system not audible in accommodation spaces.

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screen shall consist of a single screen of corrosion resistant wire of at least 30X30 mesh and shall be of such size and design as to prevent reduction in the net cross-sectional area of the vent pipe and permit cleaning or renewal. No flame screens found.

Each port light with the sill located below the margin line must have a hinged inside dead cover. Vessel does not have dead covers onboard.

A vessel must comply with applicable requirements for any radio as set forth in 47 CFR 80. Each ship transporting more than six passengers for hire equipped with a radiotelephone station must carry a radio operator who holds a marine radio operator permit or higher class license where station power does not exceed 250 watts carrier power or 1500 watts peak envelope power. Vessel master does not have a marine radio operator permit.

A vessel must comply with the requirements for station license and installation by the FCC. Vessel does not have the correct MMSI number entered in their radio.

All unfired pressure vessels must be installed to the satisfaction of the OCMI. Provide manufacturer documentation to ensure pressure vessel meets ASME or other approved standard.

Each EPIRB must be tested monthly and this test shall be logged. No EPIRB monthly tests have been logged.

A vessel on a limited coastwise route must carry six red hand flares and six orange smoke signals. Red hand flares may be substituted for orange signals. Vessel has 11 flares. Corrected on the spot.

Wooden vessel is due for fasener pull IAW COI. Complete fasener pull prior to carrying passengers to the satisfaction of the attending Coast Guard Marine Inspector.

Openings in multiple engine room power panels exposing current carrying wiring poses an electrical hazard to the crew. Properly enclose power panels to the satisfaction of the attending Coast Guard Marine Inspector (CGMI). 46 CFR 111.05-15 Old-J.

Engine room light wired using wire nuts. Make splice using mechanical type connector or place in an enclosure to the satisfaction of the attending CGMI. 46 CFR 111.50-1 Old-J

Extension cord used to plug in additional outlets and is not suitable for a fixed system. Remove or properly wire additional receptacles through breaker panel to the satisfaction of the attending CGMI. 46 CFR 111.60-5 Old-J

Engine room has multiple dead end wires. Remove wiring not in use or place in an enclosure to the satisfaction of the attending CGMI. 46 CFR 111.05-15 Old-J

Upper passenger deck not labeled with maximum number of passengers. Label the number of passengers allowed on the upper deck with at least one inch numbers and letters at the entranceway to that deck.

Missed 3rd annual inspection, past 90 days on annual inspection due 19 sep 2019.

REMOVE ALL SUSPECT AREAS AS MARKED THAT SHOW EVIDENCE OF MARINE BORER INFESTATION. AREA TO INCLUDE PLANKS ON PORT AFT QUARTER AND STBD AFT QUARTER.

MAKE PROPER REPAIRS TO ALL FRAMES RHAT SHOW EVIDENCE OF ROT OR WASTAGE.

REPLACE AREAS OF WORM SHOE THAT SHOW EVIDENCE OF WORM INFESTATION.

POST STABILITY LETTER IN WHEELHOUSE.

REMOVE UNUSED CO2 EXTINGUISHING SYSTEM.

PROVIDE VALID CERTIFICATE OF DOCUMENTATION, FCC MARINE OPERATOR'S PERMIT, AND VALID FCC COMMUNICATIONS ACT SAFETY CERTIFICATE.
