Vessel's port radar stopped rotating. The brushes were fried on the antenna motor and the unit will not rotate.

#1 SSDG overheated and shutdown. Crew found plastic bags in the strainer and believe prevented cooling water into the #1 SSDG. This caused the SSDG temperatures to shoot up high overheating it and causing a shutdown. Vessel lost power for 15 min.

NAVTEX fuse was blown. Fuse changed out and unit energized. Unit started and tested all day but vessel is not receiving messages.

The safety management system shall be maintained in accordance with the provisions of the International Safety Management Code. Over 180 items were marked as substandard during the recent dry dock inspection. The Flag Administration has identified numerous areas of non-conformity including but not limited to: A) Failure to report to nearest USCG OCMI when temporary repairs were made to a hole in the hull. B) Inadequate training for crew members authorized to use the oily water separator C) Inadequate training for correct logging of Oil Record Book Entries. D) Unauthorized alterations in engine room piping including bypass pipes between sludge tank and Dirty Oil Tank and incorrect safety relief valves. E) Substandard sanitation and accommodation for galley sinks and drains, leaking ceiling in food stores, and sewage smell throughout accommodations for months F) Substandard vessel maintenance of deck equipment, ventilation ducts, fire fighting dampers, many of the discrepancies were not properly identified or addressed G) Vessel Security Plan Control-copies of VSP were used as scrap pages in engine room An external SMS audit is required prior to departure from Singapore. Based on the number and scope of the non-conformities detected, we do not believe the SMS has been properly implemented. The scope of the audit is to determine the prevalence of non-implementation of the SMS.

Items are required to be monitored/alarmed to meet 74 SOLAS 2001, Chap II-2, Reg 52, Reg 53.4.1 & 53.4.3 as failure of these bearings led to the loss of the reduction gear on 09-10 Aug 2017.

The vessel departed Japan on July 10, 2017, bound for Concord, CA. The vessel arrived in Concord, CA on July 24, 2017 and the COI expired on June 30, 2017.
