The Rations shall be packed in permanently sealed metal containers or vacuum packed in a flexible packaging material. Vacuum seal for numerous food rations is broken and air has leaked into the food.

For tracking purposes I've dated this due for 1FEB18 as no USCG unit is currently tracking the repairs.

The main diesel engine #5 cylinder has a crack in the cylinder head and is overheating, rendering the main engine inoperable. Dead ship tow is authorized from Casablanca, Morocco to Rota, Spain, where repairs to the #5 cylinder head main engine shall be completed prior to departure. Submit RO repair report for satisfactory vessel operation to the USCG for review.

The Flag Administration has identified numerous areas of non-conformity including but not limited to: A) Failure to report to nearest USCG OCMI when temporary repairs were made to a hole in the hull. B) Inadequate training for crew members authorized to use the oily water separator C) Inadequate training for correct logging of Oil Record Book Entries. D) Unauthorized alterations in engine room piping including bypass pipes between sludge tank and Dirty Oil Tank and incorrect safety relief valves. E) Substandard sanitation and accommodation for galley sinks and drains, leaking ceiling in food stores, and sewage smell throughout accommodations for months F) Substandard vessel maintenance of deck equipment, ventilation ducts, fire fighting dampers, many of the discrepancies were not properly identified or addressed G) Vessel Security Plan Control-copies of VSP were used as scrap pages in engine room An external SMS audit is required prior to departure from Singapore. Based on the number and scope of the non-conformities detected, we do not believe the SMS has been properly implemented. The scope of the audit is to determine the prevalence of non-implementation of the SMS.
