In a ship in which fuel oil is used, the arrangements for the storage, distribution and utilization of the oil shall be as to ensure the safety of the ship and persons onboard. PSCO observed oil and oil residues in the bilges and a general lack of engine room cleanliness. SOLAS 74 (2001 cons.) II-2/15.2 60a

Resolution: Recieved class report from LR attesting to the cleanliness of the engine room/bilges. Engine room was thoroughly cleaned by ship's crew including the aft bilges where oil was originally found.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed fuel oil leaks on the MDE and the #1 and #2 generators. SOLAS 74 (2001) I/11(a) 60a

Resolved 23 August 2018
Resolution: Washers found worn on #1 and #2 generators, and were replaced. A general examination on running conditions of the #1 and #2 generators was conducted, no leaks found. A pressure test on all fuel oil and lube oil lines was conducted, no further leakages.

Resolution: Received class report from LR. Jacket water leak was found to be coming from #1 cylinder head. O ring was replaced by ship's crew, and a operational test was conducted while the main engine's jacket water pump was running, no further leaks found.

Doors fitted in boundary bulkheads of machinery spaces of Category A shall be reasonably gastight and self-closing. PSCO observed the bottom section of the door frame to the entrance of the steering gear room from the engine room was bent, causing an approximate 1 inch gap between the bulkhead and the framing, rendering the bulkhead non-gastight. 74 SOLAS (2001) II-2/47.1 17a

Resolved 23 August 2018
Resolution: Received class report from LR. C/E dismantled the bottom framing of the fire door that was found severely bent. The fitter realigned and adjusted the door frame, remounted it. Class stated there is no longer a gap and door was stated to be gastight.

The arrangement of the entrance, berths, lighting, ventilation and water supply of the hospital accommodation shall ensure the comfort and facilitate the treatment of the occupants, PSCO found strong chemical/greywater small upon entering the hospital space. The shower/tub was found to be unable to train. Crew stated that the tub piping/draining has been inoperable for approximately one week. PSCO believes tat the space is unsafe for human occupancy. Resolved 23 August 2018
Resolution: Received class report from LR. Crew dismantled the drainage cover of the shower/tub and a gamazing agent was applied, unclogging the drain. Ventilation was fitted to remove the odor. Class stated no odor was found.
