For ships constructed on or after 1 July 2014, a minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighter's communication shall be carried on board. Those two-way portable radiotelephone apparatus shall be of an explosion-proof type or intrinsically safe. Ships constructed before 1 July 2014 shall comply with the requirements of this paragraph not later than the first survey after 1 July 2018. During the fire drill attending MI’s noted that the fire lockers had designated radio charging locations, however radios were missing and not being utilized by the fire teams. Each required SOLAS fireman’s outfit shall be equipped with with an approved radio, and these radios shall be designated for fire locker / fire team use to ensure availability during any emergency onboard.

In cargo ships, all linings, ceilings, draught stops and their associated grounds shall be of non-combustible materials in the following spaces: in corridors and stairway enclosures serving accommodation and service spaces and control stations for ships where methods IIC or IIIC are specified as referred to in regulation 9.2.3.1. Structural fire protection insulation above the galley oven was removed.

Test techniques must not simulate monitored system conditions by mis-adjustment, artificial signals, improper wiring, tampering, or revision of the system unless the test would damage equipment or endanger personnel. Crew did not have testing equipment for flame detectors IAW PSTP requiring ultraviolet light source be applied. Acquire ultraviolet light source testing equipment for flame detectors.

The purpose of this regulation is to provide means of escape so that persons on board can safely and swiftly escape to the lifeboat and liferaft embarkation deck. FRP grating installed along lifeboat egress route on port and starboard embarkation decks for lifeboats #1-4. Provide evidence installed FRP gratings conform to ASTM F3059-14 standard IAW USCG CG-ENG Policy Letter 01-18.

Doors required to be self-closing shall not be fitted with hold-back hooks. However, holdback arrangements fitted with remote release devices of the fail-safe type may be utilized. Over 40 various self closing doors in class A/B boundaries had hold back arrangements or tie backs installed by the crew. Crew removed hold back ties and MIs brought to Master attention for further training and awareness.

Maintenance and inspections of life-saving appliances shall be carried out. Noted the following outstanding lifeboat maintenance items: 1. Replacement of SOLAS retro-reflective tape on all lifeboat tops 2. Reseal of deck/hull connection due to failing seams & erosion on lifeboats #2 and #4. Satisfactory maintenance to be carried out for lifeboats. 74 SOLAS (2014) III/20.3.1

No less than two lifebuoys shall be provided with self-activating smoke signals and be capable of quick release from navigation bridge. Lifebuoy not rated for bridge quick release installed on port side navigation bridge wing. Satisfactory lifebuoy with self-activating smoke signal shall be installed on port bridge wing. 74 SOLAS (2014) III/7.1.3

Ventilators shall be provided with weather-tight closing appliances of steel or equivalent material. Noted following ventilators not weather-tight: 1. Starboard bridge wing Inlet (AC910) 01-60.5-5 missing gasket 2. Emergency supply ventilator 07-61.5-2 (Port bridge wing missing gasket) Ventilators shall be provided with weather-tight closing appliances & proper gaskets. CITE - ICLL (1988) Reg. 19(4)

All life-saving appliances shall be in working order & ready for immediate use. Lifeboat & Rescue Boat medical kits discovered to be expired (Aspirin Feb 2022). Noted in authorized annual servicing report in April 2022. Satisfactory medical kits to be provided onboard survival craft. 74 SOLAS (2014) III/20.2

Condition of ship shall be maintained to remain fit to proceed to sea without danger. Noted following damage to watertight/fire doors onboard: 1. Wasted door frame MJD 04-70-5 to passage 04-58-01 (Starboard Lifeboat aft vestibule) 2. Wasted watertight door WTD 01-57-5 3. Bent stairtower door frame MJD 01-67-2 (bottom door frame aft stairtower preventing full closure) Satisfactory repairs to doors shall be made. 74 SOLAS (2020)I/11(a)

Electrical installations shall be such to maintain auxiliary services for ship during normal operation and under various emergency conditions. Discovered electrical ground on #4 fire sprinkler pump motor controls(120V AC B-phase)- via emergency switchboard. Satisfactory troubleshooting and repairs to eliminate ground shall be conducted. 74 SOLAS (2004)II-1/40.1.1 & 40.1.2

Fire protection & fire-fighting systems shall be maintained ready for use. Discovered broken pull box glass on Cargo Pump room FM200 remote cylinder controls by entrance to space. Satisfactory repairs shall be made to cylinder pull box for FM200 system. 74 SOLAS (2014) II-2/14.1.1

Each non-metallic hose must be usable for oil or hazardous material service. Wasted cargo transfer hose (SN D17495) discovered onboard. Cargo hose to be removed from service & discontinued from use. 33 CFR 154.500(c)

All ship 500 gross tonnage and upwards shall have a gyro-compass. #2 gyro-compass failed due to inoperable AC/DC power supply. Replace power supply with In-kind parts and provide proof of proper operations.

Reconnect remote operating handle for Lube Oil tank in EDG room and Brine Overboard Valve in Aux Machinery Space.

Repair damaged MCT located port side of the shaft.

Repair/Replace missing SFP insulation covering adjacent to Fire Station 101 (5-58-1)

Provide proper material to MCT along aft bulkhead of forklift storage room to meet "A" Class boundary, black cable is not tight fitting.

Provide proper metal tie downs/supports for electrical cables, locations to include: Overhead of air castle, stbd forward of flight deck and forward passageway near ammo office.

Repair or renew passageway door on 01 level aft of forklift stowage compartment, bottom is warping from kicking.
