DAY OFF

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves. The fire pump through hull sea valve was found unable to close the ingress of water.46 CFR 115.802(2)/17C.

On a ferry vessel of not more than 65 feet in length carrying not more than 49 passengers, the fire pump must be capable of projecting a hose stream from the highest hydrant, through the hose and nozzle a distance of 25 feet. Vessel is allowed to operate as a ferry vessel. Fire pump pressure relief valve was actuating with minimal pressure upon operations of the pump.46 CFR 181.300 (c) Code 17C.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Port side ventilation cover was missing means to close the damper. 46 CFR 182.465 (h) Code 17C.

Each hatch exposed to the weather must be watertight, except that the following hatches may be only weathertight. Collision bulkhead and steering compartment hatch covers were found leaking inside the spaces. 46 CFR 179.360 (a)/ COde 17C.

Each EPIRB, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required shall be logged or otherwise documented. EPIRB was not tested for two months before the exam and not properly logged as required by toe code of regulations. 46 CFR 185.728(a) & (c)/ 16C.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. FCC Radio Emergency Broadcasting placard was deteriorated and unreadable. 46 CFR 184.506 / 16C.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. Electrical connection strip located in the void 2 forward bulkhead was found lose and one cable dead ended, including all electrical connections with oxide that shall be cleaned. 46 CFR 183.200 (a)/ 16C.

Vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at all service speeds. Steering hydraulic piston ram was found leaking through its seals. Replace hydraulic steering ram, or service to replace seals, and provide evidence of servicing to USCG Marine Inspector. 46 CFR 182.610 (a)(1) / 16C.

A vessel must carry nautical chart and Current Tables. Vessel was missing above-mentioned publications. 46 CFR 184.420/ 16C.

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried that is smaller than the lower size limit of the adult life jackets. 05 child size life jackets were found color faded. Replace all color faded child-size life jackets. 46 CFR 180.71 (b)/ 16C.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and training topics. The abandon ship drills are not being conducted nor logged, and the man over board drills are not being logged according with the above-mentioned requirements. 46 CFR 185.520 (a)&(f)/ code 16C.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include the following information.(1) Date of the training; and(2) General description of the training topics. A crew training log is not being implemented or completed, and was not available for USCG inspection on board. 46 CFR 185.420/ 16C.

Watertight hatches must be marked on both sides in clearly legible letters at least 1 inch high: “WATERTIGHT HATCH—KEEP CLOSED”. Forward collision compartment and aft steering compartment watertight hatches are not marked as required above. 46 CFR 185.610/ 16C.

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. No evidence of the above-mentioned required training was provided during the exam. Evidence of mandated training shall be provided to the satisfaction of the USCG. 46 CFR 16.401(b). 16C.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Unused electrical panel found in bathroom area and dead end wires found in bathroom area and in water tank space. Repair to the satisfaction of the attending Marine Inspector. 46 CFR 183.200(b) 16(c)

All hand portable fire extinguishers shall have permanently attaches thereto a metallic name plate giving the identifying mark of the actual manufacturer. All hand portable fire extinguishers did not have the approved bracket for the extinguisher model, according to the extinguisher manufacturer. Replace to the satisfaction of the attending Marine Inspector. 46 CFR 76.50-5(d) 16(c)

A marine employer shall comply with the requirements of 4.06 in subchapter A of this chapter. The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel of each individual who was directed involved. 3 crew members and 2 tests were expired. Replace to the satisfaction of the attending Marine Inspector. 46 CFR 185.212 46 CFR 4.06-15(a)(1) 16(c)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. There was no marking for the stowage of child size life jackets and they were not separated from the adult life jackets. Mark and stow to the satisfaction of the attending Marine Inspector. 46 CFR 180.78(a)(5) 10(c)

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. Pitting found in a weld at the base of a guardrail on the top deck near the operating station. Repair weld to the satisfaction of the attending Marine Inspector. 46 CFR 177.900(b) 16(c)

At each inspection for certification and every other vessel inspection, spaces may be examined to determine that they are serviceable and in a sanitary condition. Mouse droppings discovered by water tank. Contract a company to fumigate/sanitize and provide report/cert. 46 CFR 176.818 16(c)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel missed its drydock inspection date. 46 CFR 176.600(c)(1) 17(c)

Code 07109: A pre-engineered fixed gas fire extinguishing system must Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Pre-engineer fire extingushing system failed to shut-off engines when conducting testing. 46 CFR 181.420 (a) (3).

Code 01703: The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Marine Inspector found two openings in the engine room not vapor tight so that the extinguish medium would escape, and allow air to enter into the space.

Code 07199: Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. Fuel shut off valves needed cables tightened as to close completely.

Code 04103: All cable and wire must have stranded copper conductors with sufficient current carrying capacity for the circuit in which they are used. Cable from the stbd selector switch starter has audiopipe wire installed. Does not meet marine grade standard.

Code 01305: The master of a vessel, except a vessel listed in § 185.502(a) of this part, shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. The passenger count shall be available to the Coast Guard upon request. Passenger count shall be available to the coast guard upon request. No passanger count recorded onboard the vessel.

Code 11109: The float-free link required by § 180.130(a)(1) must be: (1) Certified to meet § 160.073 in subchapter Q of this chapter, or other standard specified by the Commandant; (2) Of proper strength for the size of the life float or buoyant apparatus as indicated on its identification tag; and (3) Secured to the painter at one end and to the vessel on the other end. The lifloat painter line was found not attached to the weak link.

Code 07199: At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Engine room junction box exposed, with dead end wires in the forward engine room from engine room and aft comparment, ground wires for rudders pulled from connectors.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation. Port engine serpentine belt broke and engine overheated. The following shall be completed prior to returning to service: 1. Provide causal analysis of engine casualty 2. Provide repair plan 3. Complete satisfactory sea trial in the presence of the attending USCG Marine Inspector. 46 CFR 182.100

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. The painter must be of at least 100 feet in length, but not less than 3 times the distance between the deck where the life float or buoyant apparatus it serves is stowed and the lowest load waterline of the vessel; have a breaking strength of at least 1,500 pounds; be of a dark color if synthetic, or of a type certified to be resistant to deterioration from UV light; and be stowed in such a way that it runs out freely when the life float floats away from a sinking vessel. The port and starboard painters for the 02 required life floats were found deteriorated due to UV exposure and shall be renewed.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the the lanyard around the body of the ring life buoy. The lanyard was found deteriorated due to UV exposure and shall be renewed.

Each item of survival craft equipment must be of good quality and efficient for the purpose it is intended to serve. Each item of equipment carried must have equivalent mounting or storage arrangements that do not interfere with launching. Both installed containers for the lifefloat floating waterlight impedes removal of the waterlights and prevents the light from floating free.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The starboard side fuel shutoff valve was not able to close securely when operated.

A space containing diesel machinery must be fitted with at least two ducts to furnish natural or powered supply and exhaust ventilation. Provision must be made for closing all supply and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The installed, hinged supply and exhaust ventilation covers, had no means to secure the ventilation covers open or closed, effectively impeding ventilation to the engine space.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Inspector found the port side rudder post key allowed excessive play in the steering tie bar connection.

A vessel must comply with the applicable design, equipment, personnel, procedures, and record requirements of 33 CFR parts 151, 155, and 156. The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan is on the ship and that each person handling garbage follows the plan. Each garbage management plan must be in writing and (1) Provide for the discharge of garbage by means that meet Annex V of MARPOL, the Act, and §§ 151.51 through 151.77; (2) Describe procedures for minimizing, collecting, processing, storing, and discharging garbage; and (3) Designate the person who is in charge of carrying out the plan. A garbage management plan was not provided for the vessel.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables. Provide an up to date copy of the applicable U.S. Coast Pilot and Coast Guard Light List.

A fire pump must be capable of both remote operation from the operating station and local operations at the pump. Fire pump was not capable of starting locally.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. A guard cover was not provided for the rotating fire pump and belt.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Inspectors found three life jackets not to be in serviceable condition, requiring replacement and one missing life jacket.

All machinery and equipment must be maintained in such a manner as to afford adequate protection from causing fire, explosion. machinery failure, and personnel injury. It was discovered that the vessel was involved in a reportable marine casuatly where the starboard main engine was damaged and rednered inoperable. 46CFR182.100

Employment of an individual, or service in a position by an individual, without the appropiate license. 46CFR15.401

Failure o vessel on voyage and subject to inspection to be under direction and control of an individual with the appropiate Coast Guard license. 46CFR15.515(b)

The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Fuel Shutoff valve for the stb side fuel tank is not operable.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry—(1) Six hand red flare distress signals and Six hand orange smoke distress signals. Vessel had 4 expired flares.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Hydro static release unit for EPIRB is expired. Vessel restricted to operating within 3NM of shore until new Hydrostatic release unit for EPIRB arrives and is installed on vessel.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Exposed dead-end cables on the bridge need to be secured.

All fuel tanks must be electrically bonded to a common ground. Stbd side fuel tank was not bonded.

Replace corroded fire main piping connecting the fire pump to the fire hydrant.

(2) For a life float approved under approval series 46 CFR 160.027 or other standard. Life float is not bright orange and is missing retro reflective tape. Vessel is restricted to hold 41 passengers and 3 crew members and no more than 44 total on board. 46 CFR 180.200(C).

(A) An inspection for renewal of a certificate of inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. Vessel did not conduct an Annual inspection in the designated 90 day window, the vessel must complete a full 5 year COI to re-enter service. 46 CFR 176.404

A self priming, power driven fire pump must be installed on each vessel: Of not more than 19.8 meters (65 feet) in length which is a ferry vessel; Vessel's fire pump inoperable. Vessel restricted from carrying more than 49 pax and restricted from operating as ferry until proper operation of fire pump is proved.

A vessel must be fitted with a Federal Communications Commission (FCC) type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station if: (1) The vessel is self-propelled; (2) The vessel has a limited coastwise route; and (3) The vessel carries more than 49 passengers. Vessel doesn't have radar installed. Vessel restricted from carrying more than 49 pax without a radar.

Incorrect MMSI # input in VHF radio. Vessel must input correct MMSI # input in VHF radio.

Gauges to indicate engine jacket water discharge temperature must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Temperature gauge for stbd engine is not working and must be repaired.

Dead ended wiring under console, in steering compartment, and compartment fwd of engineroom.

Vessels of less than 50 meters in length shall exhibit an all-round white light where it can best be seen while at anchor. The pole for the anchor light was damaged during Hurricane Maria. The vessel is prohibited from anchoring at night without a suitable anchor light.

REPLACE SUBMERSIBLE ELECTRIC BILGE PUMP IN ENGINE SPACE WITH OPERABLE APPROVED TYPE.

REPLACE WING NUTS ON BATTERY TERMINAL CONNECTOR WITH PERMANENT TYPE CONDUCTORS
