The machinery, associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. Various items on the propulsion and generator prime movers must be rectified: #2 Bow Thruster - Machinery control mounting hardware Stern Thruster - Repair/replace control panel and gauges Repair exhaust leak Install drip tray under fuel filters Reinstall cover on start solenoid Splash tape - Install splash tape around the high pressure fuel piping unions on bow thrusters, stern thruster, and all three ship service diesel generator prime movers

Visible and audible alarms must be installed at the pilothouse to indicate high bilge level. Machinery room and steering gear room bilge alarms inop. 46 CFR 130.460(a)(4)

Class – 1 door may be installed at any place if the vessel’s pilothouse contains a visual indicator showing whether the hatch is open or closed. Stbd side deck escape hatch indicator is inop. 46 CFR 174.210(c)(3)

No OSV may carry flammable or combustible liquid cargoes in bulk without approval. Methanol requires a cargo tank pressure relief valve setting to be a minimum of .02 MPa (2.9 psi). The pressure relief valve setting was at 1.0 psi on the port & starboard methanol tanks. A ring buoy with light that was not intrinsically safe appeared to be within a hazardous zone of the methanol transfer area. Vessel did not have a hazardous zone diagram to confirm. Address items prior to the carriage of methanol. - 46 CFR 125.110 / CG-ENG Policy Letter No. 03-12, CH-1 / IBC Code 15.12.4

Each watertight door and hatch must comply with the requirements of 46 CFR 174. Each hatch exposed to the weather must be watertight. The vessel’s port & starboard side engine room escape hatches were showing light after and were not watertight. Crew made adjustments and go them water tight on the spot. - 46 CFR 131.893 // 46 CFR 174.220

Nothing in this part shall limit the inspector from making such inspection as deemed necessary to assure the safety and seaworthiness of a vessel. A hydraulic line for the port side steering gear had broken support brackets. The brackets were renewed on the spot. - 46 CFR 126.100

(a) When any vessel is drydocked, examination must be made of the propeller, stern bushing, sea connection, and fastenings if deemed necessary by the marine inspector. Vsl reported vibration in port mde shaft.

External high-pressure fuel delivery lines shall be double jacketed and provided with an alarm. No. 1 SSDG was in alarm for fuel line leak. Fuel was leaking from SSDG. SOLAS 14 II-2/4.2.2.5.2

Inspection and tests of main propulsion machinery shall ensure it is in satisfactory working condition. Stbd MDE would not start.

Electrical equipment must be installed to protect from electrical hazards. Open electrical junction box in fwd thruster room and hanging red alarm light in dry bulk tank space.

Portable fire extinguishers which have een discharged shall be immediately replaced. #22 extinguisher in engine room, gage indicates in the red. SOLAS 14 II-2/14.2.1.2.1.2

Ships of 500 GT and up shall be provided with an international shore connection. Connection on the vsl is not configured properly. SOLAS 14 II-2/10.2.1.7

Where pressure-indicating gauges are used, they shall be chosen to be compatible with the pressure to be indicated. In no case shall the upper range of the gauge be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function; relief device was set at 200 psi but pressure vessel only had a 160 psi gauge.

Each life buoy shall, if it is intended to operate the quick-release arrangement provided for the self-activated smoke signals and self-igniting lights, have a mass of not less than 4 kg; 02 deck buoys were used in lieu of bridge buoys for the smoke floats on the port and starboard side of the bridge. 74 SOLAS (14) III/7.1.3, LSA Code 2.1.1.7.

Each item of survival equipment with an expiration date must be replaced during the annual inspection and repair if this date has passed; smoke floats were expired.

Each connection to a conductor or a terminal part of a conductor must be made within an enclosure; #3 main propulsion engine had 02 exposed wire nuts.

Each cable and wire must be supported so as to avoid chafing or other damage; electrical cable for rescue boat davit control panel was not supported and laying on the deck.

General alarm must be audible in spaces where persons may be maintaining, repairing, or operating equipment, such as machinery spaces, bow thruster rooms, under deck passageways in cargo areas, and steering gear rooms. General alarm could not be heard in tank farm or machinery spaces.
