A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Expired required items in first aid kit, replace to the satisfaction of the attending marine inspector. 46 CFR 184.710

Bars, slats, rail courses, or an equivalent spaced at intervals of not more than 100 millimeters (4 inches). Courses were missing on the 3rd from the top making the gap more than 4", replace courses to the satisfaction of the attending marine inspector. 46 CFR 177.900(g)(iii)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of § 111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. Discovered port electric cord installed on inverter. Remove and install permanent wiring. 46 CFR 183.340(r)

At each subsequent inspection for certification, the owner or managing operator must be prepared to have the vessel ready for inspection of its fire protection equipment, including the following: Inspection of each portable fire extinguisher and fixed gas fire extinguishing system IAW NFPA 10. The (04) portable fire extinguishers and (02) pre-engineered fire extinguishing systems have not yet received annual third party servicing. Complete servicing prior to carrying passengers for hire. 46 CFR 176.810(a)(1)

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. An employer may waive pre-employment testing provided taht he or she has: During the previous 185 days beens ubject to a random testing program for at least 60 days and did not fail or refuse to participate in a chemical test. Provide proof of pre-employment testing for crew prior to carrying passengers for hire. 46 CFR 16.210(b)(2)

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces. (03) electrical wire runs in the port engine room were no longer vapor tight. Provide proof of repair to the satisfaction of the Marine Inspector. 46 CFR 177.405(e)

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. The remote fuel shutoff station labeling was missing for the port and starboard fuel tanks. Replace labeling, and provide proof ot the satisfaction of the Marine Inspector. 46 CFR 185.608

The owner or operator shall make a vessel available for drydock examinations. In accordance with U.S. Coast Guard Marine Safety Information Bulletin 09-20 "Vessel Inspections, Exams, and Documentation", the vessel's required drydock inspection has been deferred 90 days based on objective evidence provided by the owner and/or operator attesting to the vessel being fit for its intended route and service. The vessel's owner or operator shall ensure that the vessel completes a USCG Marine Inspector drydock inspection within 90 days of the drydock due date listed on the its COI. Failure to complete a USCG Marine Inspector attended drydock inspection may result in the vessel being restricted from carrying passengers.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Vessel has four high water sensors per hull but only one indicator per each hull. Vessel has bulkhead dividing each of two hulls in two sections. Provide fwd and aft indicator lights for high water alarms on each hull for a total of four and label. 46 CFR 182.530 (a)

Electrical equipment must be installed to protect from electrical hazards. Dead end wires in stbd head and stbd battery room. Remove or repair. 46 CFR 183.200

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Net that covers railing at port embarkation station had damaged clip. 46 CFR 177.900 (b)

A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight: A vessel that operates only on protected waters. Forward port bow hatch missing hardware, no longer weathertight. 46 CFR 179.360

The Certificate of Inspection must be posted under glass or other suitable material such that all pages are visible. Vessel only has 1st page of COI posted. 46 CFR 176.302

The stability letter must be posted under glass or other suitable material such that all pages are visible. Vessel only has 1st page of stability letter posted. 46 CFR 176.306

A port light must have an inside hinged dead cover regardless of whether the port light is or is not capable of being opened; Vessel's port lights dead covers are worn. Replace and repair dogs and cracked cover on port light

A hatch on a vessel that operates only on protected waters may be weather-tight; Forward starboard hatch cover is not weather-tight. Make forward starboard hatch weather-tight.

To include position information in a distress alert, the VHF-DSC radio should be properly connected to a GPS Receiver or have a VHF-DSC marine radio with integral GPS. Vessel's VHF-DSC RADIO is not properly connected to a GPS receiver.

Cooking and heating equipment must be suitable for marine use and installed in accordance with ABYC or NFPA standards;Grill is missing a locking device to prevent accidental openings during heavy seas. Grill is also missing means to collect grease or fat.

Repairs or alterations to equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI; Provide approved installation plan for grill on board vessel.

Complete Chemical Testing Program Compliance Audit.

Repair stbd side hand rails. Foundations on 2 vertical supports are loose and need to be rebedded.

Install metal fuel bowl guards on the port and stbd Racor fuel filters.

Scarf and reglass 2 areas on the port hull as identified by the CG marine inspector.

Renew access covers in the port and stbd collision bulkheads with covers of the same strength as the existing bulkhead.

Found port and stbd fuel tanks not in compliance with 46 CFR 182.440. Tank met ABYC standards for vessel with 12 pax. Vessel carries 49 pax. Replace tank to conform to same.

Worklist - The rail system does not appear strong enough throughout the periphery to withstand 200lb loads. Operator already has plans for strengthening beneath each stanchion pad to increase load strength. USCG will verify repair.

Worklist - The deadlight cover forward in the stbd hull is missing one securing handle. Replace securing handle for portlight.

Worklist - The Bomar hatches in the overhead of the cabin are cracked and crazed. Replace cracked and crazed Bomar hatches in the cabin overhead.

Worklist - Found approx 6" X 5" rotted setion of deck frame at hatch leading to the sbd fuel tank space. Make permanent repair to deck frame (Scarf 12-1)

Worklist - The emergency fuel shut-off valve handles are wasted more than 50%. Replace emergency fuel shut-off valve handles.

Worklist - The existing wood cockpit console is wood rotted throughout. Owner/Operator is replacing with a slightly smaller console of fiberglass. Provide estimate of total weight change as part of this modification. The wooden bench on the transom was wood rotted and is being replaced in kind.

Found damaged/rotted wood at the foot of the door to the port head. Repair in kind per plan.

Worklist - Found damage to the interior of the hull approx. 5 ft above the keel aft of the port head door. Repair in kind per plan.

Worklist- Found one unused valve each in the port and stbd hulls penetrating the hull above the waterline. Cap off the excess valves on the port engine fuel system supply line and above the stbd batteries.

Worklist- There is no means provided on either fuel tank to determine the fuel level within the tank. Provide means to determine the level of fuel in both port and stbd fuel tanks.

The propellers, shafts, and rudders were removed for measurement, truing and polishing. Make shafts, rudders, and propellers available for inspection.

Found deadend wires located on inboard side of Stbd engine. Remove/secure same to make safe from potential source of ignition.

Found loose connection between Emergency Fuel Shut-off remote handle and fuel shut-of valve at valve. Tighten same to ensure prompt shut-off action when used.

Found fuel supply line leading from stbd fuel tank unattached leaving line to bang against tank. Secure line to prevent excessive movement and banging.

Found loose hose on required portable bilge pump loose allowing it to come off immediately upon use. Repair same to ensure hose does not come off during operation.

Label storage of 06 additional adult PFDs to ensure easy recognition.

Found COI and Stability letter not posted for passenger review. Vessel has enclosed cabin and does not operate in harsh conditions. Post same in location easily seen by passengers.

Found excessive fasteners on emergency escape 'kick-out' window. Remove (03) fasteners, (09) total, from each vertical side and top edge of emergency escape hatch window to facilitate ease of removal in an emergency.

Found port engine RPM gauge intermittent. Prove proper and consistent operation.

Fuel hose on the stbd main diesel engine is showing evidence of dry rot.
