Except as provided by § 15.725 of this part, no vessel may be navigated unless it has in its service and onboard the crew complement required by the COI. Vessel operator had merchant marine credential expired. Vessel shall not operate unless sufficiently manned according to COI. 46 CFR 15.515

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Drug & Alcohol swabs were expired, replace to the satisfaction of the MI. 46 CFR 185.212

All cable and wire must: be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cables installations and not used as a means of support. Vessel was found with plastic supports, replace supports with metal to the satisfaction of the MI. 46 CFR 183.340 (b)(4)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Vessel's adult and kids life jackets were found stowed in the same container. Properly separate adult and kids life jackets to the satisfaction of the Marine Inspector. 46 CFR 180.78

Equipment specifications for life floats and buoyant apparatus. The equipment required for life floats and buoyant apparatus must meet the following specifications: (2) Paddle. Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. Vessel's paddles were found not lashed to the life float. Lash paddles to the life float to the satisfaction of the MI. 46 CFR 180.175(f)(2)

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Owner was unable to provide fire boy servicing report. Submit servicing report to the attending Marine Inspector. 46CFR176.810(b)(2)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: (7) A positive shutoff valve is installed at the hull penetration. Vessel is missing positive shutoff valves. Vessel must install positive shut off valves as close to the hull as possible. 46CFR182.520(e)(7)

Each vessel that operates on the high seas must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Vessel’s EPIRB pin broke which caused the vessel to place the EPIRB in a bungee cord to the side of the vessel, causing the EPIRB to not be float free. Replace the EPIRB pin creating the capability to float free. 46CFR180.64

A vessel must carry one floating water light, unless it is limited to daytime operation, in which case no floating water light is required. The vessels water light broke during man overboard drills. Vessel must replace the water light to the satisfaction of the attending Marine Inspector. Vessel limited to daytime operation until resolved. 46CFR180.70(d)

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Unless otherwise stated in this section, each item of equipment carried, whether required under this section or not, must be secured by lashings, stored in lockers, compartments, brackets, or have equivalent mounting or storage arrangements that do not: (4) Adversely affect seaworthiness of the survival craft. Vessel’s life float had a tear in the outer flotation ring creating a negative effect in the seaworthiness of the survival craft. Must repair lifeboat to the satisfaction of the attending Marine Inspector. 46CFR180.175(a)(4)

The vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel radios were not programed with the MMSI or the GPS coordinates, along with Radio telephony Certificate was expired. Vessel must renew their Radio telephony Certificate and program their DSC radios with the MMSI and GPS to the satisfaction of the attending Marine Inspector. 46CFR184.502

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first aid kit contained multiple medications that were expired. Replace all expired medication with new medication. 46CFR184.710

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels who:(1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; Vessel did not contain a testing list and records of all individuals working on the vessel. Vessel owner must include all crew members on the same list and records to the satisfaction of the attending Marine Inspector. 46CFR16.230(a)(1)

Alcohol testing (1) The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel was not carrying enough Alcohol swabs for the entirety of the crew during normal working conditions. Obtain new and an efficient number of alcohol swabs for the entirety of the crew. 46CFR4.06-15(a)(1)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel only had one portable battery light on board. Vessel owner must obtain a second portable light to meet the requirements of this regulation. 46CFR183.430

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (2) U.S. Coast Pilot or similar publication. Vessel did not have a copy of the Pilot list on board. Vessel must obtain an up to date U.S. Coast Pilot List on board. 46CFR184.420(a)(3)

The master of the vessel shall ensure that: The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel representative stopped documenting the EPIRB test back in 2019. Vessel owner tested the EPRIB in the MI presence and was educated on continuing the EPIRB log. 46CFR185.728(c)

Where rigid nonmetallic material (plastic) is permitted for use in piping systems by this section, the following restrictions apply: Through hull fittings and shutoff valves must be metalic. plastic through hulls where found on the vessel. Replace with metallic through hulls to the satisfaction of the attending marine inspector. 46CFR182.720(d)(3) Prior to next credit dry dock.

A drydock examination conducted in compliance with 176.600 must be conducted while the vessel is hauled out of the water or placed in a drydock or slipway. Propeller had damage on leading edge of 3 of 4 blades. One of the damaged blades was cracked. MI was unable to determine extent of crack due to the condition of the propeller. Vessel must repair existing damage to the satisfaction of a Coast Guard marine inspector. 46 CFR 176.610

A submersible electric bilge pump may be used as a power bilge pump if the capacity of the electrical system is designed to allow all bilge pumps to be operate simultaneously. During the inspection, a problem with the 12V DC switchboard was causing amidiships and aft bilge pumps to not operate. Proof proper operation of all installed bilge pumps to the satisfaction of the attending marine inspector. 46 CFR 182.520

Licenses for ship stations in the maritime services will normally be issued for a term of ten years from the date of original issuance, or renewal. Vessel needs to get renewed Station License and Safety Radiotelephony Certificate. 47 CFR 80.25

Owner/operator needs to update charts with Local Notice to Mariner updates. 46CFR184.420(a)(1)

Owner/Operator is not testing EPIRB monthly and not logging the tests conducted. 46CFR185.728
