The master shall ensure that the vessel's required firefighting equipment is on board in the prescribed location and always ready for use. During CO2 test of ER space with Maritime Safety Solutions, discharge pipe wastage at the base of the discharge pipe bursted releasing CO2 on deck.

In no case must the upper range of the pressure-indicating gage be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. Relief valve was set to 190 and gauge max was 200psi.

Each OSV must be placed in drydock or hauled out for examination oh the hull and the internal structural members must also be examined at the same interval to assess the safety and seaworthiness of the vessel. During this examination the following worklist items were found: - The doubler plate installation for the flukes of the anchor must be completed. - The uppermost corner bracket in the No. 2 Port Ballast tank next to the entry hatch of the space has wasted beyond acceptable limits and must be replaced. - The starboard bridge wing's support structure must be repaired due to wastage on the underside. - The starboard side air intake damper must be renewed due to wastage. - The forepeak reachrod must be made to be functional and operated in the presence of a USCG marine inspector. - The No. 8 port quick-acting fuel shutoff valve must be made to be functional and operated in the presence of a USCG marine inspector.

Each generator arranged for parallel operation must have reverse power or reverse current trips. Reverse power relay not operating as intended. Bring to proper operation. – 46 CFR 111.12-11(f)

All ships of 500 gross tonnage and upwards shall have: A gyro -compass heading repeater visually at the emergency steering position. Repeater was not functioning or cutting on. Fix to proper operation. – SOLAS (20)V / 19.2.5.2

Systems and equipment including back-up arrangements shall conform to appropriate performance standards (resolution MSC.95 (72) 3 spare bulbs not present. - SOLAS (20) V/18.2 16a

There must be ground detection for each lighting system and power or lighting distribution panel. Ground detection panel ground detection not working on multiple panels. - 46 CFR 111.05-21(c)(d)

The inspection procedures for marine engineering systems contained in subchapter F of this chapter apply. Starboard main engine non-metallic expansion joints cracked and deteriorated. Replace to good condition. - 46 CFR 126.470

The fire pump must be capable of delivering a single stream of water from the highest hydrant at a pressure of 50 psi. Could not test the main fire pump due to seafloor conditions being too muddy and fire pump unable to hold suction. ABS will test main fire pump during FMEA trials. 46 CFR 132.120(a)

Each watertight door must make proper seal with watertight gasket to be watertight. (02) Watertight door gaskets were not sealing properly as designed. Replace gaskets to make watertight.

Visible and audible alarms must be installed to indicate a fire in the engine room. Engine room fire alarm was inoperable and general alarm audible was not audible. Replace or repair.

Survival craft in a the area where it is to be launched must be illuminated by emergency power. Emergency lights by survival craft were not illuminated. Repair or replace lights as designed.

Each fixed fire-extinguishing system installed must comply with part 95. A Master or person in charge must make sure that the lock-out valve is locked open at all times. Secure valve as designed.

Each machinery space must be free of fire hazards. Stbd generator had soot and oil residue on top of cylinder head. Clean generator of soot and oil residue.

Each ring buoy must be in good material condition and serviceable. Stbd side main deck ring buoy grab line unserviceable. Repair or replace ring buoy.

Each vessel must have updated fire and safety plan in accordance with their vessel. Vessel had two emergency lights on the boat deck which were absent on the fire safety plan. Add emergency lights to fire and safety plan.

MSD Type II must be operational as designed. MSD blower was inoperable. Repair MSD to operate as designed by the manufacture's instructions.

The work rest log does not capture properly when the crew is working or resting. Log every work period even when not on watch.

The vessel had a problem with the treatment system and by-passed the MSD to the sewage holding tank without treating it. Ensure vessel treats and discharges sewage IAW 33CFR159.53

Hoses for aux engines must meet 58.60. The start air hoses for the port generator were cracked and unserviceable. Replace hose and test.

Periodic tests must be conducted at intervals specified by the Coast Guard. The PSTP’s were last conducted 3 years ago. Ensure tests are conducted and logged annually.

Provide Calibration certificate for Oil Content Meter.

Flexible F/O lines on the Port Generator not serviceable.

Vessel's motor power failure alarm on navigational bridge was inoperable. -46 CFR 58.25
