Resolution: The crew welded a metal plate over the penetration restoring the class A fire boundary. DNV-GL and USCG attended vessel and found the repair satisfactory. Class Survey report was provided attesting to satisfactory repair.

Resolution: crew removed unapproved electrical source from receptacle leading to the refrigeration auxiliary panel and a permanent electrical connection has been made from the power unit to the electrical panel. Class DNV-GL attended to satisfactory repair.
