Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 21 Marlin Type 1 life jackets installed on vessel (Model 320RT for Adults and Model 321RT for Child size) have a malfunctioning spring-loaded tab on the snap hook of the safety strap. The spring-loaded tabs fail to spring back,(return to closed position) after depressing to secure snap hook in place. Renewal required for all affected Lot #'s. Vessel has 33 serviceable adult life jackets on board, therefore passenger count shall be reduced to 31 passengers + 02 required crew, until affected life jackets are replaced with serviceable Type 1 life jackets. 46 CFR 176.808(b)

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test and make available for inspection all items in the ship's outfit, navigation lights and equipment, which are required to be carried by the regulations in this subchapter, as necessary to determine that they are fit for the service intended. Both Mast head light and anchor light inoperable during inspection. Vessel restricted to daylight hours only and shall not be remain in an anchored position until deficiency is rectified. 46 CFR 176.816

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of(50 pounds per foot) applied to the top rail in any direction. Multiple deck rail base plate screws located at port and starboard bow missing. Requires renewal and or installation. 46 CFR 177.900(b)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The vessel's stbd fuel shutoff valve was not operable. 46 CFR 182.455(b)(4)

Visual indicator must be provided at the operating station to indicate when any auto bilge pump is operating. Visual indicator at operating station not working for laz auto bilge pump. Provide operational visual indicator. 46 CFR 182.530

Vessel does not currently meet the requirements of Work Instruction 31. Mandated crew posted on COI may not enter the water or act as lifeguard until policy and procedure provided. Provide written procedure. Work Instruction 31

The selection of crewmembers for random drug testing shall be made by a scientifically valid method, such as a random number generator that is matched with crewmembers' Social Security numbers, payroll identification numbers, or other comparable identifying numbers. Under the testing frequency and selection process used, each covered crewmember shall have an equal chance of being tested each time selections are made and an employee's chance of selection shall continue to exist throughout his or her employment. Current selection process is not a scientifically valid method. Ensure random selection process complies with the above requirements.

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and Supervisory personnel must receive at least 60 minutesof training. Provide proof of completed EAP training for all personnel.

Bilge pump indicator light at helm not functioning when pump is in automatic. Make light at bilge operational for all auto bilge pumps.

Port engine would not start. Could not test Pre-engineered fixed fire fighting system. Make engine operational and test pre-engineered fixed fire fighting system.

Navigation publication not on vessel. Provide navigation publications.

Flammable materials located in forward space. Remove all flammable materials or provide approved flame locker.

The discharge ends of fuel tank vent pipes must terminate on the hull exterior as high above the waterline as practicable and remote from any hull openings, or they must terminate in U-bends as high above the weather deck as practicable and as far as practicable from openings into any enclosed spaces. Vent pipes terminating on the hull exterior must be installed or equipped to prevent the accidental contamination of the fuel by water under normal operating conditions. Diesel fuel vents onboard vessel vent into vessel.

Compass light was extinguished. Repair in accordance with 46 CFR 184.402. Vessel limited to day light operations.

High pressure fuel line clamps were loose or missing on port and stbd engines. Repair/replace in accordance with 46 CFR 176.804

VHF DSC function was not programmed. Prove DSC capability in accordance with 46 CFR 184.502.

Port fuel tank emergency shut-off did not close completely. Repair in accordance with 46 CFR 182.455.

Forward and aft engine room watertight bulkhead penetrations were not sealed. Make watertight in accordance with 46 CFR 179.360.

Port engine high pressure fuel line not clamped properly. Repair in accordance with 182.200

Fire buckets need to be replaced.

Replace oil pressure sensor.
