ORB lists weekly soundings of 5 sludge tanks (PORT/STBD sludge tanks, PORT/STBD waste oil tank, oil recovery tank) when only 2 tanks are listed on the IOPP.

(a)Observed rest/recreation room adjacent to engine room which was a CO2 protected space.
(b)Observed 4 sea chests on the vessel, only 1 was operational.
(c) Currently 4 bilge/fire pumps on the vessel, crew informed PSCO that STBD FWD pump is primary and being used. Tested fire pump STBD AFT and observed a leak of 5 gallons per minute when pump is not running.
(d) When testing PORT Emergency Generator, observed major exhaust leak.
(e) Observed multiple portable filtration systems on lube oil/hydraulic system, system not on approved piping diagram.
