The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely impacted its fitness and capability to safely maneuver the Houston Ship Channel.
Submit for review a Classification Society report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanent repairs have been completed, and that the vessel's full and normal maneuverability has been restored. (60ac)

The safety management system should include procedures ensuring that non-conformities, accidents, and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention.
Vessel was issued ISM related deficiency on 19JUN2019 during Port State Control (see form B dated 19JUN2019 for further info) with a resolution due date of next U.S. port after sailing foreign. Vessel has sailed foreign and visited two ports since issuance of ISM deficiency dated 19JUN2019, making the deficiency overdue. Non-conformity report (SMS form D05 dated 19JUN2019) generated by the ship's crew was submitted to vessel's ISM company and has yet to provide a corrective action report. An internal safety audit and corrective action report is required.
