MI observed deterioration of 3 CO2 discharge hoses coming off the cylinders. Test or replace hoses to satisfaction of USCG. Provide report of testing or bill of installation to [email withheld].

Vessel's carbon dioxide fire extinguishing system is no
longer approved. Reference MSC Letter E1-2100892 for further details. Vessel must make revisions, resubmit plans for MSC approval, and make the required modifications to the installed system within 30 days.
If this cannot be achieved within the 30 day, contact MSU Texas City Marine Inspectors as soon as possible to determine an acceptable timeline and implementation plan.

Vessel didn't have adequate lighting for the escape route from the engine room and one of the emergency lights to the aft escape wasn't operational.

Independent fuel tanks tubular gauge glasses, if fitted to diesel fuel tanks, must be of heat resistant materials, adequately protected from mechanical damage, and provided at the tank connections with devices that will automatically close in the event of rupture of the gauge or gauge lines.
The fuel tanks tubular gauges are constructed of plastic and cannot provide certification they constructed of heat resistant material.

After loading and prior to departure and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's trim and stability book, stability letter, Certificate of Inspection, and Load Line Certificate, as the case may be.
Vessel's stability letter states that the class 2 water tight door must be closed before getting underway. MI witnessed the vessel getting underway without the engine room watertight door closed.
