For the purpose of this regulation, the requirements of the code shall be treated as mandatory. PSCO observed open flame lighter being used to test flame detectors in engine room with no other approved testing equipment for flame detector onboard.

Resolution: ABS surveyor and PSC attended the vessel and verified the crews ability to test the flame detectors and with the appropriate equipment.

The arrangements for the storage, dispersion, and utilization of other flammable oils employed under pressure in power transmission systems, control and activating systems and heating systems shall be such as to ensure the safety of the ship and persons on board. PSCO found several areas where the accumulation of oil and oil leaks were present, including main engine fuel pumps 3, 4, 7, and 8, as well as, electrical wiring on main engine near cylinders 1, 2, and 3. 74 SOLAS (01) II-2/15.4

During investigation PSCO discovered a loss of main engine power momentarily occurred as a result of 25 RPM not being able to be maintained at dead slow ahead.
