Emergency lighting must be provided. Emergency lighting must provide sufficient illumination under emergency conditions to facilitate egress. Automatic, battery-operated with duration of not less than 2 hours. Light is inoperable.

Each remote valve control must be marked in clearly legible letters, at least 1 inch high, indicating the purpose and the way to operate. Placard missing.

Piping for flammable/combustible liquid must be fitted with valves clearly marked by labeling. Pipe unmarked.

The portable fire pump must be self-priming and power driven, at least 80gpm/not less than 60psi. Pump is not self-priming.

Pyrotechnic distress signals must be stowed in either a portable watertight container at the operating station, of bright color, and clearly marked in contrasting lettering with "Distress Signals"; or, in a pyrotechnic locker in the vicinity of the operating station. Flares are improperly stowed in a locker, far removed from the operating station.

The owner/manging operator must must implement a health and safety plan. The plan must document compliance with this part and include recordkeeping procedures. The owner, managing operator,or master must ensure all persons aboard comply with the health and safety plan. Limited, to no knowledge of the plan.

Safety orientations and other crew training must be documented in the Towing Vessel Record (TVR). Entry must include: date, general description, name(s) and signature(s) of individuals providing orientation/training; and name(s) of individual receiving orientation/training. No records available.

Each towing vessel must perform the applicable tests in Table 143.245(b). The tests required must be recorded in the TVR. Incomplete tests and frequency.

The officer in charge of a navigational watch must conduct a navigational assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management, and assess operational risks. To include currents, water depth, river stage, visibility and weather conditions and anticipated changes along the route, traffic, status of pilothouse instrumentation and controls, bridge air draft and other overhead obstructions, horizontal clearance and navigational hazards, lock transits, broadcast to mariners, safety/security zones, configuration of vessel/tow, and any special conditions not covered. A navigational assessment entry must be recorded in the TVR, The entry must include the date/time of the assessment, starting/ending points of the voyage or trip. No navigational assessments being conducted.

Each lifebuoy on a towing vessel must be marked in block capital letters with the name of the vessel.

Lifebuoys must have the following attachments and fittings: At least one lifebuoy must have a lifeline secured around the body of the lifebuoy. If more than one lifebuoy is carried, at least one must not have a lifeline attached. All buoys aboard have a lifeline attached.

All crewmembers must be provided with health and safety information and training to include contents of the health and safety plan, procedures for reporting unsafe conditions, use of PPE, safe use of equipment, hazard communications and cargo knowledge, use/storage hazardous materials, confined space entry, respiratory protection and lock-out/tagout procedures. Training must be conducted not later than 5 days after employment and documented in owner or managing operator's records. No record of training.

Turbo chargers must be insulated. Insulation missing @ turbo elbows. Install insulation.

Bilge alarm audible/visual @ operating station inoperable. Repair/replace as needed.

Vessel name missing on ringbuoy(s). Add vessel name.

Floating waterlight inoperable. Repair or replace with approved waterlight.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 50% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no long be permitted to operate.
