The safety management system should ensure compliance with mandatory rules and regulations. PSCO found several leaks and much pooling of flammable liquid in the engine. PSCO identified a leaking steam pipe and a broken sight glass. PSCO found damaged structural fire protection on A-60 boundary as well as an improper gasket on a firefighting nozzle. Due to several examples of poor maintenance, PSCO recommends an external ISM audit.

Resolution: Vessel provided documentation attesting to a completed external ISM audit.
The oil content meter was displaying a warning to replace the desiccator and that the oil content meter was displaying a reading of 3-4ppm while only fresh water was being flushed into the system. Unable to verify accuracy of oil content meter due to malfunctioning desiccator.
