The safety management system should ensure compliance with mandatory rules and regulations. PSCO found several leaks and much pooling of flammable liquid in the engine. PSCO identified a leaking steam pipe and a broken sight glass. PSCO found damaged structural fire protection on A-60 boundary as well as an improper gasket on a firefighting nozzle.
