Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed, and installed as to withstand the maximum working stresses to which it may be subjected. The safety management system should ensure compliance with mandatory rules and regulations. PSCO found several leaks and much pooling of flammable liquid in the engine. PSCO identified a leaking steam pipe and a broken sight glass. PSCO found damaged structural fire protection on A-60 boundary as well as an improper gasket on a firefighting nozzle. Due to several examples of poor maintenance, PSCO recommends an external ISM audit. 74SOLAS (14) IX/5.1 ISM 1.2.3.1 50ac
