The condition of the ship shall be maintained to conform with the provisions of the present regulations to ensure the ship will remain fit to proceed to sea without danger to ship or persons on board. Any defect shall be reported to the recognized organization responsible for issuing the relevant certificate. Multiple cracked transverse side-shell frames and overhead deck beams were discovered on the starboard side of the bow thruster room. SOLAS 2020 I-1/11

Periodic safety tests must demonstrate the proper operation of safety controls. The approved PSTPs call for the electrical overspeed trips on the main engines to actuate at 1035 rpm; prior to the mechanical overspeed trip at approximately 1060 rpm. However, when the overspeeds were tested on the #1 and #4 engines, the mechanical overspeed actuated prior to the electrical overspeed. Make necessary adjustments and prove proper operation of electrical and mechanical overspeeds for all 5 engines, in accordance with approved PSTPs (Rev6, section 2.2), to the satisfaction of the attending ABS surveyor.

Incinerator walls are to be protected with insulated fire bricks/refractory. There are sections of refractory inside the incinerator that require repair. Make repairs to the satisfaction of the attending ABS surveyor. UPDATE 2/23/24: The deficiency due date has been extended to 3/8/24.

Periodic safety tests must be conducted at periodical intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Per CFR regulation and CG policy, following the initial verification by the ACS and/or CG, 100 % of the PSTPs are to be tested annually by the ship’s crew, with follow-on spot checking by the ACS (ABS) and CG. The initial onboard verification of the PSTPs by CG was carried out in December 2022, and the PSTPs ultimately approved by the CG's Marine Safety Center in letter E2-2301433 of 4/17/23. At time of inspection, there was no objective evidence of annual testing of all PSTPs by the vessel crew since this PSTP approval in April 2023. Provide a corrective action plan to the CG regarding how overdue annual testing of PSTPs will be carried out, and annual cycle maintained moving forward. A copy of the CAP shall be provided to the RO that issued the SMC/DOC (ABS) for their awareness and records.

Resolution: Received satisfactory corrective action plan from port engineer. Subcom proposes to complete the PSTPs throughout the year and the crew to sign and date for each test completed on a copy of the PSTP retained onboard.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: fire detection and fire alarm systems. Various detector heads were found taped to the mounting base (reportedly due to difficulties in sourcing replacement parts for the 20+ year old system onboard) and the text on the control/display panel on the bridge is beginning to become faded/difficult to read. These issues to be addressed via repair or replacement by the next scheduled dock (2025). Code 705 = by next scheduled dry dock (2025)
