After any survey of the ship no changes shall be made in structural arrangements machinery, equipment and oher items covered by the survey without sanction of the administration. PSCO observed that the crew placed a portable fan by the #1 SSDG to help with cooling for the control panel to prevent overheating.

Satisfactory means (in the form of guard rails, lifelines, gangways, or underdeck passages, etc) shall be provided for the protection of the crew in getting to and from their quarters, the machinery space, and all other parts used in the necessary work of the ship. Outboard guard rail on port side of bay 22 is missing. Additionally, safety railings on catwalks around cargo hold 6 are damaged. ICLL 66/ Reg 25(4)

74 SOLAS (01) II-2/ 21 - Fire Extinguishing Appliances shall be kept in good order and available for immediate use at all times. The CO2 release control cabinet located in the CO2 room broke free from its mount and was held in place by CO2 activation lines. Mechanincal condition of CO2 activation system unknown.
