The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device: A self-propelled vessel of 65 feet or more in length, engaged in commercial service. AIS had incorrect Call Sign. Update AIS.

fire-extinguishing systems, fire-detection systems must be inspected or tested at least once every 12 months. No evidence that batteries replaced on detecting unit per report. Perform all req's per report with documentation.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Eyewash station bottles found expired. Replace bottles.

Windows and other openings at the operating station must be of sufficient size and properly located to provide a clear field of vision for safe operation in any condition. Window broken. Replace window.

The following alarms must be provided: (1) Main engine low lubricating oil pressure;(2) Main engine high cooling water temperature;(3) Auxiliary generator engine low lubricating oil pressure;(4) Auxiliary generator engine high cooling water temperature;(5) High bilge levels;(6) Low hydraulic steering fluid levels, if applicable; and(7) Low fuel level, if fitted with a day tank.Be visible and audible at each operating station. The alarm located at the operating station may be a summary alarm; if the alarm at the operating station is a summary alarm, the specific alarm condition must be indicated at the machinery or bilge location. Provide all alarms at operating stations including aft control.

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. Alarm setpoints: Twice every 5 years, with no more than 3 years elapsing since last test. Pressure vessel relief valves: Twice every 5 years, with no more than 3 years elapsing since last test. Provide evidence tests have been conducted.

a towing vessel with overnight accommodations and alternating watches (shift work)must have a system to detect when its master or mate (pilot) becomes incapacitated. No system installed. Install system or provide additional person in pilothouse.

The design of the fire detection system and its installation on the towing vessel is certified and inspected by a registered professional engineer with experience in fire-detection system design, by a technician with qualifications as a National Institute for Certification in Engineering Technologies (NICET) level IV fire alarm engineering technician, or by an authorized classification society with equivalent experience, to comply with paragraphs (a)(1) through (6) of this section. Provide certification.

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. Charge of storage batteries if the vessel is so equipped, for emergency lighting and power. Operator unaware of test requirements. Conduct tests per above.

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. Electronic charts not updated. Update charts regularly.

The owner, managing operator, or master of a towing vessel must ensure that the vessel's lifesaving and fire suppression and detection equipment is in good working order. Fire extinguisher bracket found broken near "forepeak" space. Replace bracket.

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crewmembers are capable of performing the duties expected of them during emergencies. Emergency instructions must be updated to new operators prescription or followed as written.

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. No towing gear record provided. Provide record.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217. Detection systems or individual detectors must be kept operational at all times when the crew is onboard. No smoke detection provided in main deck berthing. Crew unable to test per manufacturer instructions function of fire detection in other spaces including lower "forepeak" berthing. Provide detectors and test units per manufacturer instructions in presence of Marine Inspector.

Where practicable and except as provided in § 144.515, each space where crew may be quartered or normally employed must have at least two means of escape. Berthing furniture, i.e. bunks, in forepeak, blocks 2nd means of escape (scuttles and hatch). Provide 2nd means of escape.

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. Multiple highly flammable portable torches found throughout accommodation spaces. Store all flammables in approved locker/space.

The master of a towing vessel must ensure that each crew member participates in fire fighting drills and receives instructions at least once each month. Master and deckhands were unfamiliar on use of provided fire extinguishing systems. Train crew on installed/portable/semi portable systems and in accordance with revised emergency instructions. Demonstrate proficiency to USCG.

The owner, managing operator, or master of a towing vessel must ensure that the vessel's lifesaving and fire suppression and detection equipment complies with the applicable requirements of parts 141 and 142 of this subchapter and is in good working order. Liferaft rigged so as to not deploy automatically in float free arrangement, repeating improper rigging on other inspected vsls owned and operated by company. Train crew on proper rigging of liferaft.

Each lifebuoy on a towing vessel must: Be marked in block capital letters with the name of the vessel. At least one lifebuoy must have a lifeline, secured around the body of the lifebuoy. At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110. 01 lifebuoy starboard side, outside pilothouse found with lifeline not around buoy, electric light on deck without bracket, stenciling fading, and reflective tape chipped/faded. Repair all.

Piping for flammable or combustible liquid, seawater cooling, or firefighting systems must be fitted with isolation valves that are clearly marked by labeling or color coding that enables the crew to identify its function. Fuel transfer pump shutdown incorrectly identified as fire extinguishing activation point. Provide fuel piping diagram and label essential systems and controls.

Each towing vessel must be maintained and operated so as to minimize fire hazards. Temporary fiberboard panels installed overhead on ladderway entering engine room near fidley space. Remove all combustible materials from engine room.

Shaft bearing was inspected with wear readings indicating that it was worn past the manufacturer's recommended service life and must be replaced.

The Marine Inspector must examine the hull and all appendages for damage, fractures, pitting, wastage, or improper repair. The following items were noted during the latest DDX and must be corrected before resuming commercial operations. 1) overhead structure in the forepeak is excessively wasted and must be cropped & renewed. 2) Structure near the bottom of the chain locker is wasted and must be cropped and renewed. 3) vertical frame in starboard aft ballast tank is wasted and seperated from the hull and must be cropped and renewed.

The reach-rod for the forepeak is disconnected from the valve servicing the space's bilge. The owner must prove proper operation prior to departure.

Closure devices must be provided for hull penetrations to prevent water intrusion. The hatch over the steering gear space is wasted and must be replaced.

A vessel must maintain adequate structural integrity, so as not to be questioned by the OCMI or TPO. Multiple areas on the tug's hull were identified as having washed out weld, which must be gouged and filled, to the satisfaction of the attending Marine Inspector.

A towing vessel 79 feet or more in length must carry four lifebuoys that are at least 24" in diameter. One of the vessels life buoys was deteriorated and in need of replacement. 46CFR141.360(a)(3)

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light. The lights attached were not working due to expired batteries. 46CFR141.460(c)(2)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Several dead ended wires were identified throughout the vessel that must be properly secured. 46CFR143.400(a)

Each liferaft must have a painter system providing a connection between the vessel and the liferaft, and each liferaft must be arranged for float-free launching. The liferaft's painter was not properly secured to the vessels float free arraignment. 46CFR141.310 46CFR199.130(6) & (7)

The following list of items must be recorded in the TVR (logbook) (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. Vessel was not recording items properly in the TVR. Conducted some training with the vessel operator to start properly recording the items needed. 46CFR140.915(a)

Towing vessels must undergo an annual inspection within 3 months before or after the COI anniversary date. The vessel's anniversary date is 14Jul and the vessel did not conduct an annual inspection before 14Oct2021. The vessel's COI has been revoked and the vessel must conduct a COI renewal inspection prior to operating commercially in the spring of 2022.
