The PSCO found the No.2 Diesel Oil Service Tank remote safety shutoff valve to the auxiliary engines tied open with wire while the No.1 and No.2 Ship Service Generators were in operation, preventing the valve from being remotely closed in the event of a fire. The wire and the valve handle were painted the same color. The wired up valve was not approved by class.

Resolution: The wire holding open the remote fuel shut-off valve to the No.2 D.O. tank has been removed. The valve was tested for manual opening and closing, as well as remote closing by air pressure. All was found to be satisfactory.
