A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station, (1) A light to indicate discharge; (2) An audible alarm that sounds upon discharge; and (3) A means to reset devices used to automatically shut down ventilation systems and engines. The port engine discharge alarm did not indicate power or produce a visible and audible indicator of discharge. Repair and prove proper operation in the presence of the attending marine inspector. 46 CFR 181.420(b)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Man overboard drill not conducted. Conduct man overboard drill prior to carrying passengers to the satisfaction of the attending marine inspector. 46 CFR 176.808(g)

At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. Fire drill not conducted. Conduct fire drill prior to carrying passengers to the satisfaction of the attending marine inspector. 46 CFR 176.810(d)

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry, (1) Six hand red flare; and (2) Six hand orange smoke distress signals; or substitutions as per 46 CFR 180.68(c). Vessel had three hand red flares and three hand orange smoke. Provide the correct number of distress signals in accordance with the regulations to the satisfaction of the attending marine inspector. 46 CFR 180.60(a)

The platform must be suspended from the body of the float by a net or an equivalent arrangement, which when fully extended, holds the top of the platform approximately 36 in. below the center of the float body. Life float stainless steel platform frame broken. Repair to the satisfaction of the attending marine inspector. 46 CFR 160.027-3(f)

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. One paddle broke. Replace with a paddle of at least 4 feet in length to the satisfaction of the attending marine inspector. 46 CFR 180.175(f)(2) 46 CFR 180.175(f)(2)

Pendants must be fitted approximately 18 in. apart around the outside of each buoyant apparatus. Each pendant must be made up in a hank, and the hank secured by not more than two turns of light twine. Life float pendants were not made up in a hank or secured by twine. Make pendants as described by the regulation to the satisfaction of the attending marine inspector. 46 CFR 160.010-4(g)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. No emergency instructions onboard or posted at the operating station. Provide emergency instructions with the recommended format as prescribed by 46 CFR 185.512, to the satisfaction of the attending marine inspector. 46 CFR 185.510(a)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a highwater level in each of the following normally unmanned spaces, a space with a through-hull fitting below the deepest load waterline, such as a lazarette; (2) A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and (3) A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. The port side tank space high water visual alarm illuminates when the port engine space high water alarm is activated and the engine space high water visual alarm does not illuminate when activated. Make so the highwater alarm activated for that space illuminates to the satisfaction of the attending marine inspector. 46 CFR 182.530(a)

Independent fuel tanks must be located and installed as described in this paragraph (b) All fuel tanks must be electrically bonded to a common ground. The port and starboard fuel tanks are not bonded. Bond the fuel tanks as described by the regulation to the satisfaction of the attending marine inspector. 46 CFR 182.440(b)(4)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 65 feet in length carrying not more than 49 passengers, other than a ferry, provided that, (1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113. The port forward space, port tank space, and port engine room submersible electric bilge pumps are not UL 1113 approved. Provide UL 1113 approved bilge pumps for these spaces and prove proper operation to the satisfaction of the attending marine inspector. 46 CFR 182.520(e)

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Fuse blocks and fuses are of the blade type. Replace fuse holder and fuses to UL approved cartridge type to the satisfaction of the attending marine inspector. 46 CFR 183.380(l)

Each circuit breaker must meet UL 489 or other standard specified by the Commandant and be of the manually reset type designed for; (1) Inverse time delay; (2) Instantaneous short circuit protection; and (3) Switching duty if the breaker is used as a switch. The Blue Seas surface mount circuit breakers in the port and starboard engine spaces are not UL 489 approved. Replace circuit breakers with UL 489 of the same amperage to the satisfaction of the attending marine inspector. 46 CFR 183.380(m)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either, (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Starboard tank space bilge cable connections made by twisting the cable through and around a ring connector inside the enclosure. Make connection using a method as described above to the satisfaction of the attending marine inspector. 46 CFR 183.340(g)

Electrical equipment on a vessel must be installed and maintained to, (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; and (c) Minimize accidental personnel contact with energized parts. Rocker switch wired tied on outboard amidship hull in tank space is designed to be in an enclosure resulting in the electrical terminals being exposed. Mount switch in the proper enclosure to the satisfaction of the attending marine inspector. 46 CFR 183.200(b)(c)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date, U.S. Coast Pilot or similar publication. Coast Pilot expired. Provide adequate and up-to-date or extract of the Coast Pilot for the vessel’s operating area to the satisfaction of the attending marine inspector. 46 CFR 184.420(a)(2)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. (1) The starboard forward space bilge pump discharge hose is kinked potentially compromising the pump’s ability to dewater the space; (2) The port engine crankcase breather hose is almost cut in half permitting these vapors to escape into the engine space; (3) Port engine space automatic float is not mounted potentially preventing the pump from operating if the space is flooded; and (4) The sewage macerator discharge hose is kinked and burned. Repair or replace all the above to the satisfaction of the attending marine inspector. 46 CFR 176.830(a)

An EAP training program must be conducted for the employer's crew members and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. No supervisory training conducted. Provide objective evidence to the satisfaction of the attending marine inspector that an additional 60 minutes of supervisory training was conducted. 46 CFR 16.401(b)

A hatch exposed to the weather must be watertight. Gasket material for the forward port and forward starboard hatches were deteriorated.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. Hydrostatic release for EPIRB expired. Install new hydrostatic release. Vessel restricted to Lakes, Bays, and Sounds route. 46 CFR 176.808(a)(5)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. Hydrostatic release for life float expired. Install new hydrostatic release. 46 CFR 176.808(a)(5)

Child size life jackets equal to 10% of the number of persons permitted on board must be provided. Vessel needs to provide required number of child size life jackets. Vessel restricted to carrying 4 persons weighing less than 90 pounds. 46 CFR 180.71

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessel's alcohol testing strips were expired. Provide new alcohol testing strips. 46 CFR 4.06-15 (a)

Each vessel must be equipment with at least two operable portable battery lights. Vessel did not have portable battery lights. Provide 2 portable battery lights. 46 CFR 183.430

Lifelines and pendants must be as furnished by the manufacturer. Replacement lifelines and pendants must meet the requirements in Subpart 160.010 of this chapter.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Vessel had approximately 2 gallons of diesel fuel in the port engine room bilge due to a previous leaky hose on the discharge end of the fuel filter. Remove standing fuel from port engine room bilge. 46CFR176.830(b)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. Both ventilation duct closures in the starboard engine room, and the aft ventilation duct closure in the port engine room did not close completely. Make arrangements to ensure complete closure of vents. 46CFR 182.465(h)

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Starboard engine room high water level alarm light at operating station is not operational. Fix light. 46CFR182.530

An adult life jacket must be provided for each person carried on board a vessel. One life jacket had a broken buckle. Provide additional life jacket in serviceable condition. Reduce passenger count by one. Total passenger count is 41 persons until rectified. 46CFR180.71

A portable hand bilge pump must be capable of pumping water. Vessel did not have portable hand bilge pump. Acquire new portable hand bilge pump. 46CFR182.520(a)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. No separation between adult and child life jackets. Provide proper stowage of life jackets. 46CFR180.78(a)(5)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station must carry a radio operator who: holds a marine radio operator permit or higher class license. Neither the crew member nor the master onboard had a valid radio operator permit. Provide marine radio operator permit. 47CFR80.158(e)

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. Vessel did not have company’s policy regarding drug and alcohol use onboard. Place company’s policy onboard vessel. 46CFR16.401(a)

Port forward compartment hull wasted IWO the bilge pump through hole valve. Provide repair proposal for approval to the attending Marine Inspector. 46 CFR 176.802(a)(2)

Life jacket storage containers on upper deck not marked with number and identification of the items stowed inside and their size. Properly mark adult and child size life jacket stowage. 46 CFR 185.604(f)

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present to prevent the passage of vapors. The starboard aft bulkhead penetration freely communicates between the engine room and fuel tank space. Provide a proper seal to prevent the passage of vapors. 46 CFR 177.405(e)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. STBD engine room battery positive bus missing protective cover. Provide cover. 46 CFR 183.200(c)

Each buoyant apparatus must have Type II retroreflective material that extends from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. Life float had worn retroreflective material. Renew. 46 CFR 160.010-4(p)

Where a flexible vent pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. Port fuel tank vent hose application for wet exhaust and STBD fuel tank vent house application for water. Replace with correct SAE J1942 qualified hoses for marine application or SAE J1527 marine fuel hoses. 46 CFR 182.450(g)

Each vessel must be equipment with at least two operable portable battery lights. Vessels missing required portable lights. Provide. 46 CFR 183.430

A positive shutoff valve is installed at the hull penetration. Electrical submersible bilge pump for the port mid space positive shutoff valve is not installed at the hull penetration. Relocate valve as close as possible to the hull penetration. 46 CFR 182.520(e)(7)

Rigid nonmetallic materials (plastic) are not authorized in vital systems which are vital to a vessel's survivability and safety, i.e. Bilge system. Replace PVC elbow in port mid space bilge piping. 46 CFR 182.710

Vessel must be provided with satisfactory arrangement for draining any watertight compartment. Vessel was unable to demonstrate operation of the starboard forward anchor locker electric bilge pump. Prove proper operation of the bilge pump.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery. The port PSS bellow was not compressed allowing water to enter the vessel. Compress PSS bellows IAW the manufacturers bellow compression chart.

The identification tag for the life float indicating the proper strength of the link was missing. Replace float-free link with one with an identification tag IAW 46 CFR 187.137(b)(2).

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. A red hand flare distress signal may be substituted for any of the hand orange smoke distress signals. Three hand orange smoke distress signals are expired and three red hand held distress signals are expired. Vessel restricted to a Lakes/Bays/Sounds route or 30 min runs until correct number of distress flares for limited coastwise route are provided.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The EPIRB bracket is incorrect for the 2830 Globalfix V4 Gps EPIRB. Install correct bracket for the above EPIRB to the satisfaction of the attending marine inspector.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Provide a divider to separate the adult life jackets from the child life jackets in the port flying bridge storage container.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that: The pump is listed by an independent laboratory as meeting the requirements in UL 1113. Submersible electric bilge pumps are not UL1113 approved. Replace and install IAW manufacture instructions UL1113 approved submersible electric bilge pumps to the satisfaction of the attending marine inspector.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The Starboard forward, port engine room, and starboard amidship visual indicators for the automatic bilge pumps are inoperable. Prove proper operation to the attending marine inspector.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. The first-aid kit burn treatment gel, and anti-septic swabs are expired. the aspirin and eyewash are missing. Replace/add expired and missing contents to the satisfaction of the attending marine inspector.

The name of the vessel must be marked or painted in clearly legible letters on each life float paddle. Mark life float paddles with vessel name to the satisfaction of the attending marine inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible, U.S. Coast Pilot or similar publication; Coast Guard Light List; and Tide tables. Provide extracts from the publications listed above for the areas to be transited to the satisfaction of the attending marine inspector.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that a positive shutoff valve is installed at the hull penetration. The bilge overboard discharge valve at the hull penetration for the port forepeak compartment is inoperable. Replace and prove proper operation to the attending marine inspector.

Electrical equipment on a vessel must be installed to protect the vessel from electrical hazards, including fire. There are two wires under the helm on the starboard side and a power terminal at the junction box in the amidships compartment that have exposed, unsecured, leads. All three power cables shall be properly terminated or secured to the satisfaction of the attending marine inspector.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A light to indicate discharge and an audible alarm that sounds upon discharge. The audible and visual alarms for the port engine room FireBoy is inoperable. Correct to the satisfaction of the attending marine inspector.

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant [name withheld] of vessels fire buckets had a capacity of 5 gallons and missing lanyard. Drill holes to 2 1/2 gallons and attach lanyard that reaches water when on bow.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. No record of recent abandon ship, man overboard drills or training available. Conduct the above training and document for review by the attending marine inspector.

The following tests and inspections must be carried out weekly on a vessel: each survival craft must be visually inspected to ensure its readiness for use. No record of life float weekly inspections documented. Perform life float weekly inspection and document for review by the attending marine inspector.

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. The EPIRB test shall be logged or otherwise documented, as applicable. No record of monthly EPIRB test documented.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. While conducting sea trials following drydock the vessels starboard engine overheated causing the Fireboy system to activate. Demonstrate proper operation of starboard engine to the satisfaction of the attending USCG marine inspector.

The following spaces must be equipped with a fixed gas fire extinguishing system in compliance with 181.410: A space containing propulsion machinery. Fireboy for the starboard engine has been energized and shall be replaced. Provide a certified technician service report & demonstrate all shutdowns, alarms, and overrides to the attending USCG marine inspector.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing LAT/LONG for DSC operation. 47 CFR 80.905

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Vessel has audible alarm but no visual indicator for high level alarms. 46 CFR 182.530 (a)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Vessel must provide separate indicator for port forward void. 46 CFR 182.530 (c)

A vessel on which a pre-engineered fixed gas fire extinguishing systems is installed must have the following equipment at the operating station: A light to indicate discharge. Vessels discharge lights inoperable. 46 CFR 181.420 (b) (1)

46 CFR 160.010-4(g)(i): Pendants on Buoyant Apparatus shall be synthetic line or webbing type approved by its manufacturer as ultra-violet light resistant, or it is pigmented in a dark color in length of at least 12 ft long and 1/4 inches in diameter. Pendants were brittle and falling apart.

46 CFR 160.050-3(c): Grab line shall be 3/8 inch diameter suitable buoyant type synthetic material having a minimum breaking strength of 1350 pounds and resistant to deterioration from ultraviolet light. Grab lines were brittle and falling apart.

46 CFR 182.530(a): On a vessel of at least 26 ft in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each compartment. No visual alarms.

46 CFR 183.430: Each vessel must be equipped with at least two operable portable battery lights. Two lights were not working.
