The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board.
The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely impacted its fitness and capability to safely maneuver the Houston Ship Channel.
Submit for review a Classification Society report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanent repairs have been completed, and that the vessel's full and normal maneuverability has been restored.

The venting system shall be provided with devices to prevent the passage of flame into the cargo tanks. Cargo tanks 2P and 2S as well as WBT 3P, 3S, 4S had no flame screens on the venting arrangements. Cargo tanks 1P and 6P had damaged flame screens such that they would no longer prevent the passage of flame. Rectify prior to conducting cargo operations.

Resolution: Received photographs of each of the cargo tank and ballast tank venting arrangements requiring new flame screens.
