Maintenance, testing and inspections shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. Four leaks were observed on flange connections on the starboard fire main piping on deck leading to the Bosun Store Room. Continued in narrative.

After any survey ogf the ship, no change shall be made in the structural arrangements, machinery, equipment, and other items covered by survey, without the sanction of the Administration. A power cable leading to a replacement port lifeboat battery charger was found to be installed to the emergency switchboard, jeopardixing the integrity of the back stuffing tube.
