On ships carrying flammable or toxic products or both, a water spray system for cooling, fire prevention, and crew protection should be installed to cover exposed cargo tank domes, exposed on deck storage vessels, cargo liquid and vapor discharge, and loading manifolds. The system should be capable of covering all areas with a uniformly distributed water-spray. Insufficient water spray coverage was observed over tank dome #1, type "C" tanks 1 and 2 on deck, and both cargo manifolds. A rupture was also observed in the deck water spray piping leading to the port manifold. 74 SOLAS(14)VIII/13.1 IGC Code (1993)11.3.1 and 11.3.2

PSCO noted in review of SMS procedures, the testing of the related cargo systems are contradicted by the condition of systems by the attending [name withheld] verification of the vessel's SMS is required. 74 SOLAS(14)IX/3.1 ISM Code 10.1
