Periodic Safety test procedure documents approved by the Coast Guard must be retained aboard the vessel. Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm setpoints, the procedure to be followed, and the expected test result. The Company should ensure that obsolete documents are promptly removed. Vessel's PSTPs are listed as Rev. A (which is a condensed version of Rev. L), but the revision approved by the Coast Guard is listed as Rev. N. Additionally, PSTPs available on board were frequently missing the required components of a periodic safety test procedure as per 46 CFR 61.40-10(a) (i.e. missing detailed procedure to be followed and expected result). Checklists made for PSTP verification makes reference to tests not available in the current PSTP on board. 46 CFR 61.40-1(c), 46 CFR 61.40-10(a), and ISM Code 11.2

During a review of the ORB it was noted that the vessel’s crew employees the incinerator’s day tk to deal with its sludge, however the tank is not listed on its IOPP Form A. Issued (01) CG-835: MARPOL I/12 – Every ship shall be provided with tanks of adequate capacity to receive sludge. The vessel employs the incinerator day tk to manage its oily waste but it is not listed in section 3.1 of its IOPP Form A.

Vessel reported starboard shaft motor inoperable. Verify equipment is in satisfactory condition and fit for service.

Notify this office, once the repairs are made to the self reported fuel pump on the #2 cylinder of your vessel's diesel electric propulsion.

Vessel is not to get underway until the port shaft high voltage coil is repaired
