Performance of crew members assigned fire-fighting duties shall be periodically evaluated by conducting on-board training and drills to identify areas in need of improvement, to ensure competency in fire-fighting skills is maintained, and to ensure the operational readiness of the fire fighting organization. PSCO's did not observe satisfactory fire drill due to serious safety concerns.

All survival craft required to provide for abandonment by the total number of persons on board shall be capable of being launched with their full compliment of persons and equipment within a period of 30 minutes from the time the abandon ship signal is given after all persons have been assembled, with life jackets donned. During abandon ship drill ,the starboard life raft launching station was unable to demonstrate adequate proficiency with launching procedures. The crew was unable to properly secure the life raft to the embarkation deck and the launching of one life raft took approx 15 min.

General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions together with particulars of the fire detection and alarm systems the sprinkler installation, the fire-extinguishing appliances, means of access to different compartments, decks etc. and the ventilating system, including particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fans serving each section. PSCO's observed a fire boundary between the main engine room and engineering workshop that doe snot match the fire control plan.

Fire door sin main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: The doors shall be self closing and shall have approximately uniform rate of closure of no more than 40 s and no less then 10s with the ship in the upright position. PSCO's observed fire doors 4.4.156 and 4.4.170 did not close.

9900 - Safety - In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval. The alteration of the training room on DK3 has not been sent to MSC for plan review - 46CFR71.65-10(a)

9800 - Safety (hazardous) - Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. Light fixture in photo locker on deck 3 was filled with water causing sparks and smoke when turned on. - 74 SOLAS (2004) II-1/40.1.3

9900 - Safety - Above the bulkhead deck there shall be at least two means of escape. Escape door number 6.3.247 would not properly open. - 74 SOLAS (2001) II-2/28.1.2

Several bundles of wires are dead ended in the spa behind liner. properly terminate wires.

Electronic equipment being stored behind liner in the spa. Equipment not protected by fire detection or sprinklers. Equipment shall be removed or arrangement approved by administration.

Several detectors are not properly labeled. Located in galleys, pantries, and paint locker. Have Flag Administration address that all detectors in these locations are properly labeled.

Have flag administration address proper categorization of deck storage space where power amps and storage are located together.

Storage is being placed (i.e. electrical lockers, lift, machinery rooms a.c., sun compartment 3.12, etc.) Have flag administration address all spaces where storage should not be located.

Have flag administration address the non-skid located on port and stbd side boat decks. Decks are a slip hazard
