Contents of transfer procedures required by 33 CFR 155.750 must contain: (7) Procedures for topping off tanks. PSCOs attended vessel because the vessel was conducting bunker operations and during topping off 2S FO tank, approximately 125 gallons of FO spilled on deck and 40 gallons discharged into US waters. PSCOs requested topping off procedures from the CE. CE presented procedures that do not specify crew member responsibilities/roles/procedures after 90% volume is surpassed. Vessel is prohibited from conducting bunker operations until transfer procedures for topping off are provided. 33 CFR 155.750(a)(7)

The rubber matting around all the electrical panels throughout the vessel is inadequate. It does not meet the requirements of SOLAS 2001 II-1 Reg 40, 45-1.3 or 45-2. Required the company to look into this and supply rubber matting for the protection of all personnel that come into contact with any electrial panel 440V and over.
