Electrical installations shall be such that: the safety of personnel and ship from electrical hazards will be assured. PSCO observed missing doors for the breakers above the engine room space, and lack of breaker indentification on one of them. CCSSC 2.26.7.3

Life-saving appliances required by this chapter should comply with the technical specifications of the LSA Code. Except as provided by this section, all rescue boats shall comply with paragraphs 4.4.1 to 4.4.7.4. The vessel's rescue boat does not comply with specifications of the LSA Code. 1997 CCSSC 4.3.1, LSA Code 5.1.1.1

Each loading arm and each transfer pipe system, including each metallic hose, must not leak under static liquid pressure at least 1 1/5 time the maximum allowable working pressure. Crew was unable to provide proof of maximum allowable working pressure for transfer pipe system. 33 CFR 156.170(c)(4)

The survey of marine diesel engine and equipment for compliance with regulation 13 of this Annex shall be conducted in accordance with the revised NOx Technical Code 2008. The crew did not have a flag approved EIAPP certificate onboard. NOx Technical Code 2008 2.3.6

Fixed halogenated fire-extinguishing systems shall not be used in ships. PSCO's observed fixed halogenated fire-extinguishing system onboard in the engine room. CCSSC 3.8.2, 40BC

EVERY OIL FUEL PIPE WHICH IF DAMAGED WOULD ALLOW TO ESCAPE FROM THE STORAGE SETTLING OR DAILY SERVICE TANK SITUATED ABOVE THE DOUBLE BOTTOM SHALL BE FITTED WITH A COCK OR VALVE CONSTRUCTED OF SIMILAR MATERIAL TO THAT OF THE TANK DIRECTLY ON THE TANK CAPABLE OF BEING CLOSED FROM A SAFE POSTION OUTSIDE THE SPACE... PSCO DISCOVERED DISCONEECTED QUICK CLOSING VALVES CCSSC 3.20.3

ALL MACHINERY AND ELECTRICAL INSTALLATIONS MECHANICAL AND ELECTRICAL EQUIPMENT AND APPLIANCES BOILERS AND OTHER PRESSURE VESSELS ASSOCIATED PIPING SYSTEMS FITTINGS AND ELETRICAL CABLES AND WIRING SHALL BE OF A DESIGN AND CONSTRUCTION ADEQUATE FOR THE SERVICE FOR WHICH THEY ARE INTENDED. PSCO DISCOVERED WOOD PLUG IN SEWAGE TANK & UNMOUNTED SMOKE ALARMS AND A FIRE ALARM LIGHT INOPERATIVE. CCSSC 2.3

THE CERTIFICATES AND THE RECORD OF EQUIPMENT AND SHIP INFORMATION SHALL BE DRAWN UP IN THE FORM CORRESPONDING TO THE MODELS GIVEN IN THE ANNEXES. FORM E LABLED INCORRECT LIFE RAFT ONBOARD CCSSC 1.14

Lifebuoys shall be marked in block capitals of the roman alphabet with the name and port of registry of the ship on which it is carried and fitted with retro reflective material. 01 lifebuoy on the aft starboard side had no port of registry or name marked. CCSC Code 4.12.5.3

Excercises must be conducted at least once each calendar year, with no more than 18 months between excercises. Vessel was unable to show last exercise conducted in 2020. 33CFR104.230(c)(1)

Medical Certificates shall remain valid for a maximum period of two years. 02 crew members were found to have expired medical certificates. STCW I/9.5

Vessel's crew was not familiar with numerous ship systems and ship equipment to include the following: crew members were unaware the vessel had a NAVTEX or Echo depth sounder on board until a technician showed it to them, the crew was navigating on unapproved electronic equipment and not using or updating their paper charts. During a fire drill the fireman was sent to fight the fire with an empty and alarming SCBA and crew attempted to test heat detectors with smoke detector equipment. PSCOs require crew receive appropriate training. CCSS Code 5.1.7 CCSS Coode 4.17.5 CCSS Code 6.3.1

The condition of the ship and its equipment will remain fit to proceed to sea without danger to the ship, persons on board or the environment. PSCOs observed a leak in the fuel line when the rescue boat was operating in the water. CCSS Code 1.11.1

In all such ships fire control plans shall be kept up to date. Current fire control plan does not reflect an actuate amount or location of heat and smoke detectors. CCSS Code 3.13.2

All vessels shall carry up to date charts appropriate for intended voyage. Vessel was not carrying all necessary charts and not updating the ones they were carrying. CCSS Code 6.3.1

The survey referred to in 1.10.1 shall include... fire safety systems and appliances. The current supplement to the Caribbean cargo ship safety certificate does not have an accurate count of the fire detection system on board. CCSS Code 1.10.2.2

The initial or a renewal survey shall include the following; a complete inspection of the items refered to in 1.10.2.2 to ensure they comply with the requirements of this Code, are in satisfactory condition and are fit for the service for which the ship is intended. Caribbean Cargo Ship Safety Certificate and 1.3 of the Record of Equip. for CCSSC list only 6 persons for provided LSA, vessel’s LSA not accurately reflected. 3.0 of CCSSCA states an exemption Cert. has not been issued when vessel does have such certificate. Additionally, Record of Equip. for CCSSC vessel’s name is missing, number of crew accommodations is incorrect, rescue boat maker is missing, rescue boat engine is incorrect, and number of life jackets does not match with fire/LSA plan and amount on board. CCSSC Ch.1/Reg/1/10/3/3 16abc

Owner and master of every ship to which this code applies should ensure that the condition of the ship and its equipment is maintained to ensure that the ship in all respects will remain fit to proceed to sea withouth danger to the ship, persons on board, or the enviroment ship has system fault on fire alarm system. CCSSC 1/1.11.1 16AC

1282: Stairways and ladders should be so arranged as to provide, from accommodation spaces, ready means of escape to the open deck. 01 escape hatch from accommodation is inoperable. 01 escape hatch from accommodation space has been removed and is not in accordance with the fire plan. CCSS Code 3.16.4.1

0740: The condition of the ship and its equipment is maintained to confrom with the provisions of this code to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship, persons onboard or the environment. Emergency fire pump does not operate for more than 15s at a time. CCSS Code 1.11.1

0725: Fixed fire- extinguishing systems and fixed fire detection, as far as practicable and reasonable, with the requirements for these systems specified in the regulations of SOLAS 1974. CO2 system for paint locker is missing and is required in their fire plan. CCSS Code 3.8.1

1220: The ring, lines and letter shall be permanently marked on the sides of the ship to the satisfaction of the administration. ICLL Annex I/8

1299-Other (Load Lines)-The means for securing and maintaining weather tightness shall be to the satisfaction of the administration. The arrangements shall ensure that the tightness can be maintained in any sea condition. The passageway to the galley has 05 windows with no ability to close. The openings are not sealed and allow for the passage of water into the superstructure. - ICLL 66 REG 16(4)

1270-Doors-Access openings in bulkheads must be weather tight. The hatch to the paint locker has been inset and pierced. - ICLL CHII REG 12(1).

0730-Appliances (general equipment)-Every vessel should be provided with a minimum of 02 fire hoses. Vessel has 02 punctured/torn fire hoses.- CSSC 3.5.

1420-Cleanliness of engine room- Owner or master of every ship should ensure that the condition of the ship and its equipment is maintained to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship, persons on board, or the environment. Vessel had excessive oil and oily rags in the engine room bilges, creating a fire hazard. -CCSSC 1.11.1

0725- Fixed fire extinguishing installation- Fixed halogenated hydrocarbon fire-extinguishing systems should not be used in ships. Vessel is equipped with 02 fixed halon fire-extinguishing sytems. -CCSSC 3.8.2

3031-Training-Training drills should as far as practicable be conducted as if there were an actual emergency. Crew was unfamiliar with fire fighting equipment and crew has been training without donning proper protective gear and breathing apparatus. -CCSSC 4.17.2

0720-fire fighting equipment-The condition of the charges of extinguishers should be checked annually. If on checking there is any indication of deterioration, the charges should be renewed and, in any case, at least every four years. 01 fire extinguisher onboard the vessel is inoperable and needs to be recharged. -CCSSC 3.6.8.1

0700-Fire Fighting Appliances-In every ship there should be provided a sufficient number of approved portable fire extinguishers to ensure that at least one extinguisher will be readily available for use in any part of accommodation spaces. 03 fire extinguishers were missing from their labeled brackets were broken and corroded beyond being able to hold an extinguisher. -CCSSC 3.9.1

ICLL/Reg 25(4) Satisfactory means (in the form of guard rails, lifelines, gangways or underdeck passages, etc) shall be provided for the protection of the crew. Safety line missing on upper guard rails at openings to ladders and weathertight door connecting galley and crew lounge kept permanently open with a rope, which does not provide adequate protection.

STCW 95/Reg I/19(3.3) Candidate for certification shall provide proof that they meet the standard for medical fitness and hold a valid document attesting to fitness. Five of six crewmembers lack medical certificates.
