Electrical installations shall be such that: the safety of personnel and ship from electrical hazards will be assured. PSCO observed missing doors for the breakers above the engine room space, and lack of breaker indentification on one of them. CCSSC 2.26.7.3

Life-saving appliances required by this chapter should comply with the technical specifications of the LSA Code. Except as provided by this section, all rescue boats shall comply with paragraphs 4.4.1 to 4.4.7.4. The vessel's rescue boat does not comply with specifications of the LSA Code. 1997 CCSSC 4.3.1, LSA Code 5.1.1.1

Each loading arm and each transfer pipe system, including each metallic hose, must not leak under static liquid pressure at least 1 1/5 time the maximum allowable working pressure. Crew was unable to provide proof of maximum allowable working pressure for transfer pipe system. 33 CFR 156.170(c)(4)

The survey of marine diesel engine and equipment for compliance with regulation 13 of this Annex shall be conducted in accordance with the revised NOx Technical Code 2008. The crew did not have a flag approved EIAPP certificate onboard. NOx Technical Code 2008 2.3.6

Fixed halogenated fire-extinguishing systems shall not be used in ships. PSCO's observed fixed halogenated fire-extinguishing system onboard in the engine room. CCSSC 3.8.2, 40BC

PSCO DISCOVERED DISCONEECTED QUICK CLOSING VALVES CCSSC 3.20.3

ALL MACHINERY AND ELECTRICAL INSTALLATIONS MECHANICAL AND ELECTRICAL EQUIPMENT AND APPLIANCES BOILERS AND OTHER PRESSURE VESSELS ASSOCIATED PIPING SYSTEMS FITTINGS AND ELETRICAL CABLES AND WIRING SHALL BE OF A DESIGN AND CONSTRUCTION ADEQUATE FOR THE SERVICE FOR WHICH THEY ARE INTENDED. Lifebuoys shall be marked in block capitals of the roman alphabet with the name and port of registry of the ship on which it is carried and fitted with retro reflective material. 01 lifebuoy on the aft starboard side had no port of registry or name marked. CCSC Code 4.12.5.3

Excercises must be conducted at least once each calendar year, with no more than 18 months between excercises. Vessel was unable to show last exercise conducted in 2020. 33CFR104.230(c)(1)

Medical Certificates shall remain valid for a maximum period of two years. 02 crew members were found to have expired medical certificates. STCW I/9.5

Vessel's crew was not familiar with numerous ship systems and ship equipment to include the following: crew members were unaware the vessel had a NAVTEX or Echo depth sounder on board until a technician showed it to them, the crew was navigating on unapproved electronic equipment and not using or updating their paper charts. During a fire drill the fireman was sent to fight the fire with an empty and alarming SCBA and crew attempted to test heat detectors with smoke detector equipment. PSCOs require crew receive appropriate training. CCSS Code 5.1.7 CCSS Coode 4.17.5 CCSS Code 6.3.1

The condition of the ship and its equipment will remain fit to proceed to sea without danger to the ship, persons on board or the environment. PSCOs observed a leak in the fuel line when the rescue boat was operating in the water. CCSS Code 1.11.1

In all such ships fire control plans shall be kept up to date. Current fire control plan does not reflect an actuate amount or location of heat and smoke detectors. CCSS Code 3.13.2

All vessels shall carry up to date charts appropriate for intended voyage. Vessel was not carrying all necessary charts and not updating the ones they were carrying. CCSS Code 6.3.1

The survey referred to in 1.10.1 shall include... fire safety systems and appliances. The current supplement to the Caribbean cargo ship safety certificate does not have an accurate count of the fire detection system on board. CCSS Code 1.10.2.2

The initial or a renewal survey shall include the following; a complete inspection of the items refered to in 1.10.2.2 to ensure they comply with the requirements of this Code, are in satisfactory condition and are fit for the service for which the ship is intended. Caribbean Cargo Ship Safety Certificate and 1.3 of the Record of Equip. for CCSSC list only 6 persons for provided LSA, vessel’s LSA not accurately reflected. 3.0 of CCSSCA states an exemption Cert. has not been issued when vessel does have such certificate. Additionally, Record of Equip. for CCSSC vessel’s name is missing, number of crew accommodations is incorrect, rescue boat maker is missing, rescue boat engine is incorrect, and number of life jackets does not match with fire/LSA plan and amount on board. Owner and master of every ship to which this code applies should ensure that the condition of the ship and its equipment is maintained to ensure that the ship in all respects will remain fit to proceed to sea withouth danger to the ship, persons on board, or the enviroment ship has system fault on fire alarm system.
