Under all sailing conditions, including manoeuvring, the speed, direction of thrust and, if applicable the pitch of the propeller shall be fully controllable from the navigation bridge. When sailing outbound on the Lower Mississippi River, the vessel reported a loss of primary pitch control from the navigation briedge. Every oil fuel pipe, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space in the event of a fire. -Fuel oil shut-off valve did not trip on first attempt; 2nd and 3rd attempts were sucessful, and the team noticed an audible air leak from valve.

Means shall be provided whereby normal operation of propulsion machinery can be sustained. The vessel was not able to get underwaydue to a faulty relay between bridge and engine control causing the CPP to pitch to 50% ahead on its own under bridge control.

Piping systems & fittings shall be adequate for the service intended. The mechanical seal for the vessel's A/C cooling pump has failed. The crew rigged a fire hose from the fire & G/S pump suction piping to the A/C cooling pump discharge piping to provide A/C for the crew. The rigging (fire hose) was installed on 30APR18 & poses a threat of flooding to the crew and ship.

PIPING SYSTEMS SHALL BE OF AN ADEQUATE DESIGN AND CONSTRUCTION. PSCO OBSERVED PATCH ON BILGE/BALLAST EDUCTOR PIPING BETWEEN GATE VALVE AND SHIP'S SIDE SHELL IN WAY OF PORTSIDE BOTTOM DECK OF ENGINE ROOM.
