Ships SMS maintenance form 3.5.3 were completed on 30JUL2022 stating all relesing controls were in the proper position and readily accesible for immediate use. no records of non-conformitis were reported. As delinated in defeciency 2. The ship failed to identify a non conformity during routine maintenance of the fixed CO2 fire fighting system is required. 74 SOLAS (20) IX/3.1 705a,c

2-07109 - Fixed fire extinguishing installation- Fire protection systems and fire fighting systems and appliancces shall be maintained ready for use. PSCO observed type "A" CO2 cylinder valve with the safety pins inserted in 20 engine room co2 bottles, preventing the activation of the CO2 system in the event of a fire in the engine room. 74 SOLAS (01) II-2/14.1.1 60a,c

03105 - Covers - The means for securing and maintaining weathertightness shall be to the satisfaction of the administration. The arrangements shall ensure that the tightness can be maintained in any sea condition. PSCO noted that the hatch cover to the CO2 room was no longer weathertight because the handle had been cut off. ICLL 66 Annex 1/16(4) 17ac

131990-Other (Machinery) - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO noted an oil leak on the valve supplying the circulating pump in the purifier room. 74 SOLAS(14) I/11(a) 17ac

06199 - Other (Cargo) - The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO was notified that during the port state control examination, a hydraulic cylinder return line burst on the forward crane. Cargo operations were halted. 74 SOLAS (14) I/11(a) 17ac

Under all sailing conditions, including manoeuvring, the speed, direction of thrust and, if applicable the pitch of the propeller shall be fully controllable from the navigation bridge. When sailing outbound on the Lower Mississippi River, the vessel reported a loss of primary pitch control from the navigation briedge. PSCO observed that the main pitch controller on the navigation bridge in inoperable. 74 SOLAS (97) II-1/49 (60ac)

74SOLAS(04) II-1/26.1: The nachinery accociated piping systems and fittings shall be of a design and construction adequate for the sevice for which they are intended. PSCO observed an active hydraulic leak on the STBD side of the Coops & Nicborg gantrycrane. 13199

PSCO observed excessive corrosion and a 1'x2' hole covered with plastic and duct tape in the emergency fire pump space tank ventilation intake ventilation duct. Recommend the intake ventolation duct to be replaced prior to loading danderous cargo. 07116

Every oil fuel pipe, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space in the event of a fire. -Fuel oil shut-off valve did not trip on first attempt; 2nd and 3rd attempts were sucessful, and the team noticed an audible air leak from valve.

Means shall be provided whereby normal operation of propulsion machinery can be sustained. The vessel was not able to get underwaydue to a faulty relay between bridge and engine control causing the CPP to pitch to 50% ahead on its own under bridge control.

Piping systems & fittings shall be adequate for the service intended. The mechanical seal for the vessel's A/C cooling pump has failed. The crew rigged a fire hose from the fire & G/S pump suction piping to the A/C cooling pump discharge piping to provide A/C for the crew. The rigging (fire hose) was installed on 30APR18 & poses a threat of flooding to the crew and ship.

PIPING SYSTEMS SHALL BE OF AN ADEQUATE DESIGN AND CONSTRUCTION. PSCO OBSERVED PATCH ON BILGE/BALLAST EDUCTOR PIPING BETWEEN GATE VALVE AND SHIP'S SIDE SHELL IN WAY OF PORTSIDE BOTTOM DECK OF ENGINE ROOM.

0611. 17ac. The normal equipment of every lifeboat shall consist of a food ration for each person. Lifeboat is rated for 26 people and only contains 11 rations. 78SOLAS 14CONS CH-III-34/LSA CODE CH. IV 4.4.8.12

On ships carrying at least two search and rescue locating devices and equipped with free-fall life boats one of the search and rescue locationg devices shall be stowed in a free fall lifeboat. 9900. 17c. Any vessel that is equipped with ballast water tanks and bound for ports in the United States must submit a ballast water report no later than 6 hours after arrival. Vessel did not report ballast to NBIC. 33CFR151.2060b(3)

9900. 17c. Nontank vessels may not operate upon the navigable waters of the United States unless in compliance with a vessel response plan approved under 33CFR155.5065. Vessel does not have an approved vessel response plan. 33CFR155.5021

9900. 10c. Vessel owners and operators must prepare and submit notice of intent unless a waiver is granted by EPA. Vessel is operating without an EPA vessel general permit and failed to subit an NOI. VGP Part 1.5.1.1 appendix E- NOI

To discharge ballast water into the waters of the United States, the vessel must either ensure that the ballast water meets the ballast water discharge standard as defined in 33CFR151.1511(a), use an AMS or ballast exclusively with water from US public water system. Vessel shall not discharge water into waters of the United States until this requirement is met. 33CFR151.1512

1740- 15 PPM OCM- The oil record book indicated a malfunction on 15 PPM alarm on 24NOV13. C/E was unaware of what the malfunction was or if repaired. In addition C/E and crew unfamiliar with procedures to recall OWS history. Provide Corrective action plan and clarification from certificate issuing authority.- MARPOL 74/78 Annex I/11

0950 - Electric equipment in general - All electrical apparatus shall be so constructed as so installed as not to cause injury when handled or touched in the normal manner. PSCO observed the 440v electrical connection on deck for the gantry crane was found to have exposed wiring which in turn could be hazardous to the ship’s crew. SOLAS 09 II-I(45)

1499 - Other (prop and aux machinery) - Every ship shall be provided w/ a main steering gear to the satisfaction of the admin. Ship’s crew was unable to operate steering in remote operation from the nav bridge. In PSCO opinion, remote steering needs to be certified by a technician. SOLAS II-I (29)

1330 - Winches and Capstans - The condition of the ship and its equipment shall be maintained to ensure that the ship in all respects will remain fit to proceed to sea. PSCO observed one capstan missing and one capstan inoperable (does not rotate). PSCO recommends vessel provides documentation attesting the proper repair & functionality of both capstans. SOLAS 09 I/II

0199 - Other (certificate) - Each transfer pipe system must not leak under static liquid pressure at least 1.5 times the max allowable working pressure. Vessel could not provide documentation of liquid testing of fuel oil transfer pipes or cargo pipes. PSCO recommends that vessel provide test records to Coast Guard prior to bunkering or conducting bunker operations in the U.S. 33 CFR 156.170

Have Flag review minimum safe manning to increase engine room watch by one. Currently rotate a two person watch, not adequately fulfilling rest periods, need for possible additional person. STCW VIII/2

Ship's Cook to provide documentation of cooking and victual preparation qualifications dual "AB- COOK" functions to be reviewed for possible rest period violations and conflicts. STCW VIII/2

Vessel has one immersion suit for all crew onboard however number of suits is not reflected in the SOLAS safety equipment certificate. SOLAS I/12

Fire hose station midship-port side as identified on the vessel's approved fire fighting plan was missing. No hose, nozzle, spanner wrench, or means of stowing equipment.
