Where remote control of propulsion machinery from teh navigation bridge is provided and the machinery spaces are intended to be manned, the following shall apply: automation systems shall be designed in a manner which ensures that threshold warning of impending or imminent slowdown or shutdown of the propulsion system is given to the officer in charge of the navigational watch in time to assess navigational circumstances in an emergency. In particular, the systems shall control, monitor, report, alert and take safety action to slow down or stop propulsion while providing the officer in charge of the navigational watch an opportunity to manually intervene, except for those cases where manual intervention will result in total failure of the engine and/or propulsion equipment within a short time for example in the case of overspeed. PSCO reviewed alarm history for the MDE and found that the #1 cylinder had a high temperature alarm warning at 437 deg celsius, 454 deg celsius and slow down activation simutanesously; which resulted in a slow down immediately wihtout a chance from the navigation officer to override the alarm during a passing arrangement while pilot was on board. Ships crew could not provide full ahead for a significant amount as requested by the pilot. 74 SOLAS (a) 2014 Con II-1/31.2.10

Special consideration shall be given to the malfunctioning of the hydraulic, pneumatic or electrical means for control in main propulsion machinery including controllable pitch propellors. PSCO observed control air dryer bypassed and not in operation. No mention of dryer in starting procedures for MDE. Chief Engineer stated it should be in operation and not bypassed. The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The design shall have regard to materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. PSCO observed a main diesel engine jacket water cooling system was actively leaking from cylinders #3 and #6. SOLAS 74 (2014) II-1/26.1 60a/c

10 - Safety of Navigation › N/A - No Subsystem › Bridge operation

Steering gear control shall be provided: Where the main steering gear is arranged in accordance with paragraph 6. By two independent control systems, both operable from the navigation bridge. This does not require duplication of the steering wheel or steering lever. Where the control system consists of a hydraulic telemotor, a second independent system need not be fitted, except in a tanker, chemical tanker, or gas carrier of 10,000 gross tonnage and upwards. While conducting Steering test from helm station on bridge. PSCO identified that while operating in hand mode on No.2 system, the steering only operates to the maximum of 30 degrees to PORT and 32 degrees to STBD. All other systems were tested Sat.

Resolution: Received class report from ABS clearing this deficiency. Rudder fixed.
