The portable fire pump must be self-priming and power-driven, with: A minimum capacity of at least 80 gpm at a discharge gauge pressure of not less than 60 psi, measured at the pump discharge. The vessel is equipped portable fire pump was found to only supply a maximum of 52 psi. 46 CFR 142.325(e)(1)

Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. All Lifejacket lights were found to be expired as of 02/2025. 46 CFR 141.340(g)(1)

Each towing vessel must carry at least one appropriately sized lifejacket, approved under approval series 160.002, 160.005, 160.055, 160.155, 160.176, or 160.255 for each person on board. Vessel was equipped with work vests that did not meet the required approval series. 46 CFR 141.340(a)

Towing machinery used to connect the towing vessel to the tow must be suitable for its intended service. It must be capable of withstanding exposure to the marine environment, static and dynamic loads expected during intended service, the towing vessel's horsepower, and arrangement of the tow. Stanchion support for towing line winch has wastage and must be addressed. Provide repair proposal plan with timeline. 46 CFR 143.460(c)

Machinery systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. When conducting steering tests from hard PORT to hard STBD, the Steering gear was aggressively hitting the PORT rudder stop. When steering to the STBD side the gear was using the transom to stop the rudder and was a good distance from the STBD rudder stop. Provide a statement from a P.E. attesting to the condition of the vessel’s steering system. 46 CFR 143.205(a)

Electrical conductors passing though watertight bulkheads must be installed so that the bulkhead remains watertight. Vessel has multiple holes/penetrations in the engine room watertight bulkhead that appears to have been used for electrical cable. 46 CFR 143.400(h)

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. The bilge has an excessive amount of oil in it; the oil must be removed and provide a statement of where that oil came from. 46 CFR 142.220(a)

Installed generators and motors must have a data plate listing rated kilowatts and power factor (or current), voltage, and rated ambient temperature. The 7KW generator in the engine room had no nameplate data plate, nameplate data plate must be provided. 46 CFR 143.55(b)(2)

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. This subpart summarizes the responsibilities of credentialed mariners, marine employers, MRO, SAP and other chemical testing service providers in 49 CFR part 40. The regulations in 49 CFR part 40 should be consulted to determine the specific procedures which must be established and utilized. Drug testing programs required by this part must use only drug testing laboratoriescertified by the Department of Health and Human Services (DHHS). Vessel did not have a drug testing program present, must provide a program. 46 CFR 16.113(a)

Each lifebuoy on a towing vessel must: Be marked in block capital letters with the name of the vessel. Both lifebuoys were found to be marked with the wrong vessel’s name. 46 CFR 141.360(b)(4)

The owner or managing operator must implement a health and safety plan. The health and safety plan must document compliance with this part and include record keeping procedures. Health and safety plan was unable to be provided for inspection. 46 CFR 140.500(a)

Each ship subject to the Bridge-to-Bridge Act must have on board a radio operator who holds a restricted radiotelephone operator permit or higher class license. Operator did not have a valid operator permit. 47 CFR 47-80.163

The master of each towing vessel must keep an accurate list of crewmembers and their assigned positions and responsibilities aboard the vessel. List of crewmembers and their assigned positions were not aboard the vessel.

Recordkeeping. Records of drills and instruction must be maintained in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel. There were no recent entries (2024+) located in the TVR. 46 CFR 140.420(g)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Electrical connection for the angle indicator comes into contact with the steering ram, which may cause it to be damaged/broken. 46 CFR 143.400(a)

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. Exposed rotating pieces of machinery on aft end of main engine. 46CFR144.820

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Steering linkage loose and not properly secured to hydraulic ram. 46CFR143.205

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel's name and hailing port not marked on the hull of the ship. 46CFR67.123(a)

At least one lifebuoy must have a lifeline, secured around the body of the lifebuoy. If more than one lifebuoy is carried, at least one must not have a lifeline attached.

2nd Generator must have a reliable means of notification when an emergency condition exist to include low lube oil pressure, and high jacket water temperature.

There must be an installed or portable bilge pump for emergency dewatering. Vessel must demonstrate proper operation of fixed bilge pump in engine room space.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Connections must be hard wired to batteries in engine room space.

Fuel systems for towing vessel main engines and generators must have proper operation. Vessel must cap terminal end of fuel tank ventilation line to ensure closed piping ventilation arrangement.

Provide proper stowage and arrangement for survival craft as required by 46 CFR 141.310.

Provide 01 additional immersion suit. 46 CFR 141.350 (a)(2)

Provide primary lifesaving for vessel in accordance with table 141.305. 46 CFR 141.305

Provide signage IVO engine room general alarm flashing light as required in 46 CFR 143.235. 46 CFR 143.235

Replace or repair fire hose with defect near coupling connection to pump resulting in leak. 46 CFR 142.325 (d)

Permanently mount fuel filter for forward generator. Fuel filter is currently zip tied to cooling water piping. 46 CFR 143.205

Repair wire splice for bilge pump in engine room. Wiring is currently spliced utilizing a wire nut, connection must be of a permanent type. 46 CFR 143.400 (f)

Provide documentation showing fire alarm system is approved and functionality tested to the requirements of 46 CFR 142.330 (a). 46 CFR 142.330 (a)

Label pyrotechnic distress signal container as "DISTRESS SIGNALS" in contrasting 1/2 inch lettering as required by 46 CFR 141.375 (e) (2)

Repair and demonstrate proper operation of amperage meter on switchboard for aft generator. Ensure proper operation of voltage meter for aft generator labeled as inoperable. 46 CFR 143.400 (a)

Mark lifejackets, immersion suits and life ring buoy with new name of vessel as required in 46 CFR 199.70. 46 CFR 199.70

Mark with block capital letters on stowage locations for lifejackets, immersion suits and work vests are marked with number in each stowage location in addition to required markings to indicate the item the stowage location contains. Intructions for marking locations indicated in 46 CFR 199.70 for lifejackets and immersion suits. Vessel's forward hatch to the foc'sle is completely ceased unable to open. Make compartment available for inspection.

Vessel does not have Type I PFD's onboard. Vessel required to a have total of four.

Multiple electrical hazards are present including exposed wiring and faulty grounding arrangements.

Vessel's fuel filters in the engine room do not have flame impingement bowls. Install properly onboard IAW 46 CFR 143.260 (c)

Vessel does not have fuel system maintenance plan. Provide and place onboard IAW 46 CFR 143.255.

Provide and place onboard required navigational day shapes IAW 46 CFR 140.720.

Provide proof of documentation for crew training (non crewmember orientation) IAW 46 CFR 140.410 & 46 CFR 140.415.

Provide proof of oil transfer procedures IAW 33 CFR 155.730.

Prove proper operation of the portable fire pump and its equipment IAW 46 CFR 142.325(e).

Properly stow the portable fire pump with its hose and nozzle outside of the machinery space IAW 46 CFR 142.325(f).

A supply of clean fuel filters must be onboard IAW 46 CFR 143.255.

Properly install bilge alarms audible & visual IAW 46 CFR 143.275.

Properly repair conenction points to the MSD. The piping appears to be separating IAW 33 CFR 159.7.

Properly install and demonstrate operation of low level alarm on steering gear system IAW 46 CFR 137.220(d)(1).

Demonstrate satisfactory steeering gear test IAW 46 CFR 137.220(d).

All watertight doors need adjustment on top section not enought contact IAW 46 CFR 144.330. Watertight door to engine room has excessive gap on top section when closed and secured.

The owner or operator must complete a load analysis that shows the electrical power source is sufficient to power the sum of loads. A record of analysis must be retained by the owner or operator IAW 46 CFR 143.555(b)(7).

Installed generators and motors must hvae a data plate listing rated kilowatts and power factor or current, voltage and rated ambient temperature IAW 46 CFR 143.555 (b)(2).

Generators must be provided with overcurrent protection no greater than 115 percent of their rated current and utilize a switchboard or distribution panel IAW 46 CFR 143.555 (b)(3).

Ensure alarms and monitoring are provided for as outline in 46 CFR 142.230. Main engine and auxiliary generator must show alarm conditions for low oil pressure and high cooling water temperature at operating station, if summary alarm is shown at operating station, the specific alarm condition must be shown at machinery location.

Emergency lighting must be provided for all internal crew, working & living areas. Emergency lightin sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either automatic battery operated with a duration of at leasts two hours or non-electic phosphorent adhesive lightin stips that are installed along escape routes and sufficiently visible to enable egress with not power IAW 46 CFR 143.410 (b)(1)(2).

Each towing vessel must be quipped with at least two portable battery powered lights one must be located in the pilothouse and the other at access to the engine room IAW 46 CFR 143.410 (c).

Verify work vests meets approval 46 CFR 160.053 IAW 46 CFR 140.450.

Each storage container containing a work vest must be marked "work vest" IAW 46 CFR 140.430(b).

Provide complete health and safety plan & recordkeeping procedures set forth in 46 CFR 140.505 IAW 46 CFR 140.500.

The skiff is accepted as the survival craft for the towing vessel ensure all of 46 CFR 141.330 is included in the craft IAW 46 CFR 141.330.

Storage locker for flammables required and must meet 46 CFR 142.225.

Ensure crewmembers participate in fire drills and receives instruction at least once a month IAW 46 CFR 142.245.

Properly/ permanently install semi-portable currently lying on the engine room deck IAW 46 CFR 142.315(i).

Piping and machinery exceed 428 degrees including fittings, flanges, valves exhaust and turbochargers must be insulated IAW 46 CFR 143.220.

Fuel shutoff requrements each remote valve control must be marked clearly legible letters one inch high indicating purpose of valve and why to operate it IAW 46 CFR 143.255.

Prove / demonstrate proper operation of the remote fuel shutdowns IAW 46 CFR 143.255.

Recommend preserve aft engine room and winch appears to have wastage IAW 46 CFR 144.200.

Machinery spaces must be separated by bulkheads doors may be installed provided they are self-closing type IAW 46 CFR 144.410. Door between engine room and berthing area must be self closing.

Waste receptacles must be noncombustible and with no openings IAW 46CFR 144.425.

Protection against hot piping each exhaust pipe from an internal combustion engine which is withing reach of personnel must be insulated or otherwise guarded to prevent burns IAW 46 CFR 144.830.

Guards in dangerous place. An exposed hazards such as gears and rotating machinery must be protected exapme cover, guard, or rail IAW 46 CFR 144.820.

Verify proper electrical installtion for vessel including 110V insalled in junction box at engine room overhead. Also 110V battery charger plug make/ install to prevent backing out IAW 46 CFR 143.400.

Verify piping arrangement that is dismantled and disconnected that its is properly blanked off and prevent water ingresses/ flooding IAW 46 CFR 143.250. Verify and provide information showing that all unused through hull penetartions in engine room are permanently sealed.

Verify keel cooler arrangement & installation and prove whether or not valves required.

General alarm system does not have a flashing red light IAW 46 CFR 143.245.

Porthole window starboard side engine room is broken/ cracked. Replace or repair per manufacturer specifications.

The machinery must be equipped with gauges at the machinery locations see regulation for all required IAW 46 CFR 143.230(c). Gauges must be provided at machinery location for main engine and all auxiliary generators indicating RPM, oil pressure and engine cooling water temperature.

Each towing vesel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Verify contents of first aid kit 160.041.
