MARV auxillary setting devices were switched on the pilot valves on cargo tank dome #3, according to vessel's "set and seal" certificate from DNV-GL. Name/Data plates were missing on cargo tank #3 forward valve and cargo tank #1 aft valve. A gas carrier shall comply with the requirements of the IGC. The changing of the pressures and setting of the alarms should be carried out under the supervision of the master and in accordance with procedures approved by administration and specified in the ships operations manual. The safety relief valve auxillaries on tank #4 were swapped and not in accordance with DNV-GL Relief valve document dated 2018-03-12. Additionally,the cargo control system was set to "at sea" mode while operating with "In harbor" settings. Ther are no procedures detailing the valve locations or to let crew know to change cargo control system settings.

Vessel has not procedures detailing the valve locations or to let crew know to change cargo control system settings in deficiency (01). Crew did not follow the approved method of testing the oily water separator in deficiency (02). Recommend the vessel receive an external audit within 30 days.
