The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Vessel P&S tank spaces have new wires run though old cable penetrations and the fire block is missing, must replace to make water and fire tight.

The CGMI feels this will not be possible in all working conditions as observed during recent inspection using 3 crew members.

Shutoff valves, installed so as to close against the fuel flow, must be manually operable. Following the vessels ship yard maintenance period, the fuel shutoff valves were improperly re-installed and do not allow for full closing of shutoff valve. A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space.
