Except for hatches between cargo, special category, store, and baggage space, and between such spaces and the weather decks, all openings shall be provided with permanently attached means of closing which shall be at least affective for resisting fires as the divisions in which they are fitted. PSCO observed temporary opening in bulkhead between F. & E. Store and main vertical zone in the F. & E. Store across from ship's office. 74 SOLAS (01) II-2/30.1

Resolution: Vessel submitted DNV Class survey report attesting to the rectification of the opening found in the upper deck No.1 store room bulkhead. The opening was closed with insulation and fully repaired.

Whenever a defect is discovered which effects the completeness of a ships equipment, the master or owner shall report at the earliest opportunity to the administration or RO responsible for issuing relevant certificates. PSCO observed ships generator number one was inoperable due to failed mechanical seal causing fresh water to mix with lubricating oil. Vessel chief ordered parts to repair but had not informed Class or Flag. 74 SOLAS (20) 1/11(c)

PSCO observed fire door to ropes room in a Class A boundary being held back with a manual back hook. 74 SOLAS (01) II-2/30.4.4

An immersion suit of an appropriate size complying with the requirements of Section 2.3 of the code shall be provided for every person on board the ship. If a ship has any watch or work stations which are located remotely from the place or places where immersion suits are normally stowed, additional immersion suits of an appropriate size shall be provided at these locations for the number of persons normally on watch or working at those locations at any time. The vessel did not have immersion suits provided for bridge watchstanders on the bridge. 74 SOLAS 20 III/32.3.3

The master, owner, operator, agent, or person in charge of a vessel must submit a ballast water report to the NBIC by electronic ballast water report format using methods specified at NBIC's website. For any vessel bound for U.S. ports and places outside the Great Lakes and Hudson River must submit the report no later than 6 hours after arrival. Vessel's crew was not aware of the requirement to submit an NBIC report even though the vessel did not, and was not preparing to, submit an NBIC report. Vessel submiotted NBIC report after PSCE instructed them to do so. 33 CFR 151.2060(b)(3)
