PSCO observed exposed dead end wires from multiple light fixtures throughout the engine room and steering gear room. Wires were protruding out of enclosure with plastic wire nuts. PSCO also observed dead end wires in steering gear room.

The company and the ship shall comply with the requirements of the International Safety Management (ISM) Code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which maybe established by the company.
1. Vessel has not been following procedures outlined within the Safety Management System (SMS) for verifying expiration dates of lifeboat equipment; PSCO identified 9 out of 30 food rations with expiration dates of 2019.
2. Vessel failed to make proper notifications to the Coast Guard COTP of sick crew on board. Vessel was aware of two crew members with COVID-19 like symptoms and failed to report them for two days while in the Sector Delaware Bay COTP Zone. Vessel's SMS clearly states to report immediately to the USCG COTP any crew who show signs and symptoms of COVID-19 or flu like illness. Vessel had ample opportunities to notify the Coast Guard but proper notifications were never completed.
Prior to the release of the detention, an audit of the vessel's safety management system must be completed.

To assist in casualty investigations, ships, when engaged on international voyages, subject to the provisions of regulation 1.4, shall be fitted with a voyage data recorder (VDR) as follows: ships, other than passenger ships, of 3,000 gross tonnage and upwards constructed on or after 1 July 2002. The VDR should continuously maintain sequential records of preselected data items relating to the status and output of the ship's equipment, and command and control of the ship...The voyage data recorder on the ship cannot turn on and is not operational. The exact time that the voyage data recorder became inoperable is unknown.

Except as where otherwise stated, the normal equipment of every lifeboat shall consist of a food ration totaling not less than 10,000 kJ for each person the lifeboat is permitted to accommodate. 9 out of 30 food rations in the freefall lifeboat expired in February of 2019.
