2020-Fire drills-Fire drills should be planned insuch a way that due consideration is given to regular practice in the various emergencies that may occur depending on the type of ships and the cargo. PSCO observed that the last three (03) fire drills took place in the galley. When asked to perform drill in galley, crew was not able to perform drill adequately. 74SOLAS14 III/19.3.5.1. 17a.

2020-Fire drills-Each fire drill shall include checking of fireman's outfit and other personal rescue equipment. PSCO ran fire drills twice and firemans' outfits were not checked prior to fireman entering the drill. Fireman sent into the drill with the mask on and no air and there were manufacturer's stickers inhibiting the view from each face mask. 74SOLAS14 III/19.3.5.2.3, 17a.

2099-Other (SOLAS operational Def.)-Where "A" class divisions are penetrated for the passage of pipes, arrangements shall be made to ensure that the fire resistance is not impaired. PSCO observed sewage and gray water hoses passing from the engine room to the steering gear room, where emergency fire pump is located, propping doors open and negating A-60 boundary. 74SOLAS01 II-2/18.1.1, 10c.

Resolution: Crew routed hoses alternately from the sewage discharge on the main deck the aft peak tank. This allowed doors between the engine room and the emergency fire pump space to properly close.
