The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. Objective evidence indicates that the company has failed to ensure proper reporting procedures and preventative maintenance including, but not limited to, failure to follow NTVRP reporting procedures, failure to document unapproved modification to OWS, excessive oil accumulation in the engine room, multiple lifebuoy lights extinguished, and cargo securing procedures not being followed. An external audit shall be carried out by the administration or the RO prior to the ship being released from detention.
