Inert Gas System- The system shall be capable of delivering inert gas with an oxygen content of not more than 5% by volume to the cargo tanks at any required rate of flow. PSCO spent over two hours waiting for the crew to show proficiency and ability to reduce the oxygen content below 5%. Crew must demonstrate full system capabilities including ability to maintain output of less than 5% oxygen content, and proper shutdowns and alarms. Rectify deficiency prior to conducting any/all cargo operations.

On the #2 cargo oil pump, two temperature sensor housings were not properly installed with a gas-tight gland preventing accumulation of vapor in the housing. Lowest temperature sensor on #2 COP was not properly installed allowing it to be pulled straight out of the pump housing. Rectify deficiency prior to conducting cargo discharge operations.

15 - Safety Management Systems (ISM) › N/A - No Subsystem › Shipboard operations

The company should establish procedures, plans, and instructions including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship, and protection of the environment. Vessel crew demonstrated substantial unfamiliarity with the oxygen analyzer and how to accurately sample oxygen content in the produced inert gas. It took the entire engineering staff over two hours to demonstrate the system's ability to produce less than 5% oxygen. The crew had to remove the three sample lines in the inert gas supply main to verify which one was active. Erroneous sampling lines and valves from the replaced O2 analyzer resulted in further confusion. Lack of familiarity and systematic ineptitude resulted in the crew damaging filters and blocking sample lines. Require external audit to verify proper operational instructions are implemented and the crew is properly trained on how to operate the inert gas system.
Recommend removing erroneous equipment and piping to reduce confusion and prevent future crews from improperly operating a vital cargo system.
