01-74SOLAS(01)II-2/15.2.5 - 16ac- Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage tank shall be fitted with a cock or a valve on the tank capable of being closed from outside the space the tank is stored. While conducting a test of the emergency generator, PSCO observed activation of fuel shut-off valve. Valve failed to stop flow of fuel to emergency generator.

Resolution: Class sent survey report attesting to the surface cracks that was temporarily repaired by deck personnel. Class has imposed a recommendation requiring the vessel to permanently repair the cracks within a three month period.

1410-Propulsion Main Engine-Number 1, 2, and 3 pistons were found to have excess fuel and lube oil leaks on the fuel injector, puncture valve fittings, the low pressure fuel pump drives, and the actuator for the exhaust valve. The number 4 cylinder jacket has a minor water leakage. Vessel is to repair engine to the satisfaction of class. SOLAS 74 CONS ED 2004 REG 26.1
