01-74SOLAS(01)II-2/15.2.5 - 16ac- Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage tank shall be fitted with a cock or a valve on the tank capable of being closed from outside the space the tank is stored. While conducting a test of the emergency generator, PSCO observed activation of fuel shut-off valve. Valve failed to stop flow of fuel to emergency generator.

Resolution: The valve was overhauled by shore side vendor and placed on board the day of Class inspection 09 FEB 2016. The emergency diesel generator fuel shut-off valve was tested without remark.
