The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all aspects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed 3 inch by 18 inch hole in hull on starboard side between frames 36 and 34 with damage to the frames. Vessel is restriced to Grandview Anchorage pending a repair plan for hull damage approved by the US Coast Guard

The condition of the ship and its equipment shall be maintained to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship. During operational testing of steering gear, PSCO noted that steering pump #2 had hydraulic fluid leaking iwo the seal between motor and pump. In addition, hydraulic fluid accumulated under both #1 and #2 steering pumps, at least 1-2 cm in depth. The fixed storage tank sight glass indicated the tank level was approximately 6 cm low. C/E states pump #2 needs to be rebuilt and requested rebuild kit 3 months prior and it was received on board prior to PSCO exam. 74 SOLAS (14) I/11(a)

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. When conducting operational testing, PSCO noted no water was coming out of cooling discharge outlet as required by manufacturer's instructions. C/E stated he believes the impeller is not functioning properly creating lack of cooling water. 74 SOLAS (14) III/20.2

The angular position of the rudder shall be indicated on the navigation bridge and be recognizable in the steering gear space. PSCO noted that rudder angle indicators on bridge, bridge wings, steering compartment indicators, and on the steering gear itself showed a difference of 4-5 degrees depending on location. 74 SOLAS (04) II-1/29.11

The machinery and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. PSCO observed "M/E Cylinder Oil" tank in A/C room leaking oil from piping flange beside quick closing valve. Crew applied soft patch of gasket material and hose clamp and plugged drain with rags painted to match deck. 74 SOLAS (04) II-1/26.1

Drills shall be recorded. PSCO noted discrepancies of logged drills for the months of Aug 2019 and Oct 2019 indicating previous drill write ups were used. Documentation shows fire drills as being conducted at exact same date/time in two different locations onboard and a third drill write up for the same day shows third location (24Aug19). Fire drill and rescue drill logs from 13Oct19, 24Aug19, and 14Sep2019 have different locations and start times but narrative is the same for each event. These similarities give the appearance to PSCO that documentation was written but drills were not conducted as described. 74 SOLAS (14) III/19.5

Each loading arm and each transfer pipe system including each metallic hose, must not leak under liquid pressure at least 1.5 times the maximum allowable working pressure. Vessel's bunker line was tested with air only. Provide documentation attesting to a satisfactory liquid test of the vessels bunker line IAW 33 CFR 156.170 prior to bunkering in the U.S.
