The design, construction, workmanship and arrangement of main propulsion machinery must at least be equivalent to the standards established by the ABS Steel Vessel Rules. Port side generator RPM gauge was found inoperable. Repair PRM gauge within 30 days.

Each OSV must carry at least one rescue boat. Outboard engine pull start cable broke. Engine must be repaired and a man overboard drill witnessed by a CG Marine Inspector prior to movement of the vessel.

Liftboat jacking systems are vital systems and must comply with Sections 4/1.13.1 through 4/1.13.3 of the ABS's “Rules for Building and Classing Mobile Offshore Drilling Units” as well as meet the applicable requirements of part 128 of this subchapter. Liftboat jacking system experiencing low pressure on one leg. Proceed to Cameron, LA as planned for further troubleshooting and repairs to the satisfaction of the attending Marine Inspector. Repairs and final inspection and testing are to be performed prior vessel going back to offshore service.

Each vessel shall have a red light on the port side showing an unbroken light over an arc of horizon of 112.5 degrees and so fixed as to show the light from right ahead to 22.5 degrees abaft the beam on its respective side; port running light was not visible when viewed from a distance off the port beam due to an antenna blocking it.

Annual inspection shall be made by a MI to determine vessel is in safe & seaworthy condition to include watertight integrity. During aft fire pump test, MI witnessed water coming from pinholes in deck above starboard black water holding tank. When hatch to tank was opened, MI discovered void was full of water. Vessel rep and master suspect damaged firemain piping caused flooding. Vessel master shall dewater space and gas free prior to entry for assessment and repairs. Current repairs include approximate 6” X 6” deck insert, new gasket for void hatch and firemain piping renewal. Vessel shall notify MI of damage extent and provide repair proposal before repairs commence. MI to attend vessel on 23Mar2021 to inspect space for extent of damage and completed repairs. Vessel not authorized movement until repairs complete to satisfaction of MI and shall remain jacked up during this period.

Remove plastic fuel filters and replace with metal cups on M/E and Generators

Replace non-operational cooling water temperature gauge on Port Generator

Place exposed deadend wires in junction box or remove completely

Provide current charts.

Provide current Coast Pilot

Provide new impulse rocket charges for the line throwing impulse rocket.

Provide Emergency lighting sufficent to allow passangers and cre to finid the way to survival craft, muster stations and embarkation systems.

Provide a crane certified from an approved cerfying authority.

Renew retor reflective tape on ring buoys

Log monthly EPIRB tests & record lifesaving maintenance checks & corrections.

Remove NAVTEX from bridge or make operational.

Update chart 11340

Renew or remove old E. Generator discharge valves on CO2 system.

Mount placard directing persons from CO2 remote discharge station to CO2 room in case of remote failure.

Install self-closing device on stbd engine room door.

Renew gauges on both submersible fire pumps.

Replace missing rescue boat equipment IAW 46 CFR 133.175 chart.

Provide updated charts

Missing valid FCC Bridge to Bridge Certificate

Ring buoy compromised in way of of foam fill hole, unknown quainty of water in ring buoy. - Replace

Fire plan outdated, fix and resubmit to USCG for approval

Provide placards for general alarm in both engine rooms.

Provide two buoyant projectiles for line throwing guns, required to have 4, only 2 onboard.
