Launching and embarkation appliances complying with the requirements of section 6.1 of LSA Code: Where davit arms are recovered by power, safety devices should be fitted which will automatically cut off the power before the davit arms reach the stops in order to prevent overstressing the falls or davits. Limit switch for port lifeboat is inoperable.

While entering port at 0500 vessel slowed speed and RPM's were unable to maintain during the slower speed causing the main diesel engine to shutdown. After multiple attempts to keep the engine running the pilots anchored the vessel at anchorage B-11.
