Every company should develop, implement, and maintain a safety management system which includes the following functional requirements: instuctions and procedures to ensure safe operation of ships and protection of the enviornment in compliance with relevant international and flag State legislation. PSCO observed the crew was not follwing their VMS cargo procedures. The loading procedures require a person to man the manifold at all times during cargo operation, and the PSC team saw no crew member manning the manifold upon arrival to the vessel. Additionally, Part A 10 of the vessel's ship to shore checklist was acknowledged that "scuppers on board are effectively plugged and drip trays in position are empty" despite excessive leak of the manifold containment drain. Provide corrective action plan within 30 days with concurence from Certificate Issuing Authority ensuring proper cargo transfer procedures and documentation are being maintained. Solas 78 (a) 2014 (cons) Ix/3.1 ISM Code 1.4.2

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have a mechanical means of closing each drain and scupper in the container or enclosed deck area required by this section. PSCO observed gasket leak on the containment drain. 33 CFR 155.310 (a)(3)

All steering gear components and the rudder stock shall be of sounds and reliable construction to the satisfaction of the Administration. Special consideration shall be given the suitability of essential componenet which is not duplicated. The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the sea or persons on board. The means for securing and maintaining weathertightness shall be to the satisfaction of the Administration. #1 HFO tank (port side) manhole cover bolts and securing hardware (16 of 26 bolts and nuts) have wasted away. ICLL I/16(4)

74 SOLAS (2014) IX/3.1 The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. PSCO identified leaks on the main engine, service generators, in the purifier room, and in cargo pump room, detailed in deficiency #2, #3, #4. As stated in the SMS under General FFE Maintenance Process listed in section 9.5.1.3, "the Master is responsible for reporting defects to the Company immediately." The crew failed to report these items as required by the SMS. These identified deficiencies are evidence that the ship and/or company are not meeting the SMS requirements. 74 SOLAS (2001) II-2/62.8.1 Special consideration shall be given to the design and location of scrubber and blowers with relevant piping and fittings in order to provent flue gas leakages into enclosed spaces. The inert gas blowers have multiple holes in the casing. 17ac

PSCO identified A-class doors that were incapable of closing in several locations. 10c

74SOLAS (2001) II-2/4.2.2 In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. PSCO identified active fuel leaks on the #3 ship's service diesel generator. 1/2 inch deep of fuel oil and lube oil was identified on top of the cylinder valves. Fuel and lube oil was draining to the bilge beneath the generator and had pooled to a depth of approximately 3 inches beneath the generator. - 30abc

74SOLAS (2001) II-2/4.1.2 The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to limit the accumulation of flammable vapours. PSCO identified evidence of active leaks from the cargo stripping pump in the cargo pump room. Crew rigged hoses to drain product from the stripping pump to 5 gallon buckets in the bilge of the cargo pump room. PSCO identified partially filled 5 gallon buckets of product, pooled product and cargo residue in the cargo pump room flat. - 30abc

74SOLAS(2001)- II-2/4.2.2 In a ship in which oil fuel is used, the arrangements for the storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. PSCO identified active fuel leaks from the fuel oil transfer pumps in the purifier room. Additionally, crew had rigged catchment basins beneath the transfer pumps to capture leaking fuel oil. 30abc

No person shall conduct an oil or hazardous material transfer operation unless: all connections in the transfer system are leak free. On 05SEP18, the vessel experienced a discharge of product during cargo transfer operations due to a pinhole leak in the MARPOL line (between frames 69 to 71). 33CFR156.120(p) The vessel shall not use the Marpol line while in U.S. waters until repaired and the deficiency is cleared by the USCG.

1230-Satisfactory means shall be provided for the protection of the crew in getting to and from their quarters, the machinery space, and all other parts used in the necessary work of the ship. Found multiple areas of wastage on catwalks that pose a hazard to the crew. icll66, Reg.25.4 50ac

When emergency source of electrical power is a genrator, it shall be driven by a prime mover with an independent fuel supply and automatically start. During test of EDG, start failed due to an abundance of water in fuel tank. Water was drained and EDG started after 30 minutes of correction.

Vessel's X Band radar is not operational; the vessel is required to have a secondary radar vessel is required to have repairs carried out prior to departure to the satisfaction of the USCG.

Vessel does not have an immersion suit for 2-1/2 year old passenger onboard vessel. Provide an immersion suit of adequate size and maneuverability for passenger prior to next US port after sailing foreign to the satisfaction of USCG.

Fuel oil supply lines do not have screens or otherwise suitable protection to avoid oil spray or oil leakage onto the sources of ignition. Provide proper screens or protection to fuel oil supply lines prior to next US port after sailing foreign to the satisfaction of Class.

Cage for explosion-proof in emergency power source space (Located on the bridge) is broken in 3 places. Replace cage prior to next US port after sailing foreign to the satisfaction of Class.

While testing the IG system it was observed that water was leaking from around the caseing gasket for the #1 and #2 blowers.

WHEN TESTING IG SYSTEM IT WAS OBSERVED THAT INERT GAS WAS LEAKING ARROUND THE CASEING GASKET FOR #2 BLOWER AND AT #2 BLOWER DISCHARGE EXPANSION JOINT, POSING AN OXYGEN DEFICIANT ATMOSPHERE HAZARD IN THE UPPER ENGINE ROOM.
