Manholes and flush scuttles in position 1 or 2 within superstructures other than enclosed superstructures shall be closed by substantial covers capable of being made watertight. PSCO observed No. 6 cargo hold tank was hissing and allowing air to escape. Ensure the hatch is watertight and not allowing the escape or entrance of any material. Every company should develop, implement, and maintain a safety management system which includes the following functional requirements: instuctions and procedures to ensure safe operation of ships and protection of the enviornment in compliance with relevant international and flag State legislation. PSCO observed the crew was not follwing their VMS cargo procedures. The loading procedures require a person to man the manifold at all times during cargo operation, and the PSC team saw no crew member manning the manifold upon arrival to the vessel. Additionally, Part A 10 of the vessel's ship to shore checklist was acknowledged that "scuppers on board are effectively plugged and drip trays in position are empty" despite excessive leak of the manifold containment drain. Provide corrective action plan within 30 days with concurence from Certificate Issuing Authority ensuring proper cargo transfer procedures and documentation are being maintained. 74 SOLAS (2014) IX/3.1 The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should ensure that the policy is implemented and maintained at all levels of the organization, both ship-based and shore-based. PSCO identified leaks on the main engine, service generators, in the purifier room, and in cargo pump room, detailed in deficiency #2, #3, #4. As stated in the SMS under General FFE Maintenance Process listed in section 9.5.1.3, "the Master is responsible for reporting defects to the Company immediately." The crew failed to report these items as required by the SMS. These identified deficiencies are evidence that the ship and/or company are not meeting the SMS requirements. Fuel and lube oil was draining to the bilge beneath the generator and had pooled to a depth of approximately 3 inches beneath the generator. - 30abc

On 05SEP18, the vessel experienced a discharge of product during cargo transfer operations due to a pinhole leak in the MARPOL line (between frames 69 to 71). 33CFR156.120(p) The vessel shall not use the Marpol line while in U.S. waters until repaired and the deficiency is cleared by the USCG.
