The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. VOM-OSPO19 pumproom entry procedure ventilation is started at least (1) hour prior to the programmable entry and during the works. As denoted in deficiency #2, the cargo pump room #2 fan was wired improperly during vsl visit to Freeport, Bahamas on 11/28/2019. As such, the pump room was supplied with positive as opposed to negative {suction type} causing a hazardous condition. Pump room was entered multiple times by the vessels crew since the last service. {74 SOLAS 14 IX/3.1} {Ref ISM Code 10.1}

Cargo pump rooms shall be mechanically ventilated and discharges from the exhaust fans shall be led to a safe place on the open deck. The ventilation shall be of the suction type using fans of the non-sparking type. #2 Pump room exhaust fan was wired backwards causing the pump room space to be positivley ventilated as opposed to the required suction type. Electrician swapped the wiring allowing the ventilation fan to spin in the direction necessary to provide negative (suction type) ventilation. An additional internal verification of the vessel's SMS focusing on maintenance of the ship and cargo pump room entry procedures is required in three months. 74 SOLAS (01) II/2.59.3.1
