Conditions of the ship and its equipment shall be maintained to conform with regulations to ensure ship remains fit for sea without danger to ship or persons onboard. Following items require maintenance on cargo deck: 1. Missing containment plug by deck water seal 2. Deteriorated electrical cabling fitting by deck water seal 3. Disconnected grounding cable (8P overhead piping by centerline) 4. New flame screen #6S Aft WBT vent 5. Corroded electrical conduit ivo #2S Foam Hose station 6. Missing supports for shower piping (P, C, S) 7. Corroded electrical conduit box (Stbd manifold, aft #5S) 8. Safe access to bow houses filled with equipment 9. Replace PV breaker TLI sight glass 10. Replace #1C tank cover gasket 11. Corroded aft #1C IG inlet electrical conduit 12. Replace broken latch on swing door (Port Bow) 13. Air leak ivo fwd stbd anchor deck - air line connection to deck underneath grating 14. Mechanical plugs missing at multiple containment boxes Repairs due within 30 days to satisfaction of RO. Life-saving appliances shall be in working order and ready for immediate use. Medical kits for ship's liferafts expired June2022 and annual liferaft servicing not due until September2022. Spare medical kits to be provided at muster location for expired medical items. Mitigation plan to be submitted and servicing completed within 30 days to replace expired items. Life-saving appliances shall be in working order and ready for immediate use. Liferaft painter line incorrectly attached to the vessel, not the weak link. Ship fixed during inspection and verified by USCG [name withheld] - 74SOLAS(2014)III/20.2

Rescue Boats-Before the ship leaves port and at all time during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. 01-Observed rescue boat davit limit switch missing, follow risk mitigation plan. 02-improper penetration & loose wire connection IWO winch. 03-unserviceable retro-reflective tape STBD side of Rescue boat. One or more pressure/vacuum breaker devices shall be provided to prevent the cargo tanks from being subject to a positive or negative pressure. PV breaker water is low showing inaccurate pressure reading of IG line and not providing protection as designed. SOLAS 74a (04) II-2/11.6.3.4 50a

In tankers, electrical equipment, cables and wiring shall not be installed in hazardous locations unless it conforms with standards not inferior to those acceptable to the organization. Found Pressure gauge junction boxes unsecured and holed for: 1 P/S, 3 P/S, 4 P/S, 8 P/S. SOLAS 74a (04)ll-l/45.11

An immersion suit of an appropriate size complying with the requirements of section 2.3 of the Code shall be provided for every person on board the ship. Vessel conducting seam test of immersion suits at three year intervals, majority of suits manufactured in 2005 and are over ten years of age. For immersion suits over 10 years of age shall conduct seam test at intervals not exceeding one year. 74 SOLAS(2004)III/32.3.3 MSC/Circ.1047 10c

On a tankship, each cargo tank must be equipped with an overfill device. Found 7S COT overfill alarm was active and could not be reset, verified tank level (Cont) below 98%. Vessel is restricted to filling tank no greater than 95% until repaired. 33 CFR 155.480 705a

Satisfactory means shall be provided for the protection of the crew in getting to and from all other parts used in the necessary work of the ship. Found Catwalk structure forward of pump room to be severely wasted, holed and unsafe for use. Repair prior to use. ILLC 66 Annex I/16 705a

dentify “TANK OVERFILL ALARM” in black letters at least 2" high on a white background. Vessel tank overfill alarm signage was faded and red on white. 46CFR 39.2007(d)(2) 50a

The lights and shapes specified in these rules shall comply with the provisions of Annex I. Found Christmas tree lights housing with water and one light out. 72 COLREGs Rule 20 50a

All ships shall have; a receiver for global navigation satellite system. Found GPS antenna mount to be unsecured and loose. 74 SOLAS (14) V/19.2.6 50a

Each life raft or group of life rafts shall be stowed with a float-free arrangement. Observed 01 life raft with the HRU not secured to cradle. SOLAS 74a (04)III/13 10c

Prior to conducting cargo loading operations replace teh pressure vent valve for cargo tank number 4 also excercise and repair the vaccum valve on cargo tank number 6. 74SOLAS(14) II-2/6.1

Illustrations and instructions in appropriate languates shall be conspicoulsy displayed at muster station for the method of donnign lifejackets. SOLAS (14) III/8.4.3

The emergency stop located at the port mainfold shall bomply with cables and wiring installed in hazardous zones, this items shall be rapired prior to cargo opearations. The enclosure box is wasted away and should be replaced in the next 30 days. SOLAS II-1/45.11

Cylinders must alwways be scured and when onot in use must be stowed in a rack in an upright position, with the valve protection cap in place. The argon gas for calibraiton for the flue gas system was being stowed strapped to handrail in the upper engineroom. 46CR47.60

The turbochargers on both No. 1 and 2 Generators are missing the lagging around the exhaust side of the turbo leaving high temperature surfaces exposed. SOLAS II-2.2.6

The bilge holding tank at frames 13-17 suffered a 1.5cm hole in the hull plating. Temporary plate and cement patches were applied. The crew shall monitor the condition of the patch and report any change of conditions to the USCG. Permanent repairs are are due during the next drydock.

Make necessary repairs to vessel's hydraulic ram on the free fall lifeboat davit. Repairs and operational testing to be completed to the satisfaction of OCMI prior to departure.

Make repairs to #1 circ pump discharge outlet pipe.

Make operable #1 center cargo tank high pressure alarm.

Make operable the port slop tank level indicator.

Rescue boat steering was found faulty during routine maintenance . Worn out plastic cable guide. Secondary steering (Tiller} installed tested and demonstrated to crew .

Replace old, fuel soacked lagging on fuel supply line beneath deck plating on all ship service generators and beneath the deck plates on both fuel oil purifier units.

Properly install lagging on #2 ship service diesel generator exhaust elbow

Properly install lagging on piping near ship service diesel generator fuel oil boost pumps leading to the filtering unit.
