Prior to getting underway for the first time on a particular towing vessel, each crewmember must receive a safety orientation. The Master could not provide proof of completion of a safety orientation for the crew. Conduct safety orientation with the crew and document completion of the same in the TVR or official logbook including the information required by 46 CFR 140.410(d)(1)-(4).

The Master of a towing vessel must ensure that drills are conducted and instructions given to ensure that all crewmembers are capable of performing the duties expected of them in an emergency. No records of drills and instruction were provided. Conduct the applicable drills referenced in 46 CFR 140.420(d) with the crew.

The VSO must ensure that a security drill is conducted within one week of the vessel's reactivation following an out of service period. No vessel specific security drills have been conducted. Conduct security drills with the crew.

The date and time that a navigation watchstander, including master, officer in charge of a navigational watch, and lookout assumes a watch and is relieved of watch must be recorded in the towing vessel record or official logbook. If an engineering watch is maintained, comparable records documenting the engineering watch are required. The vessel is not properly logging change of watch. Update change of watch procedures to ensure all required items are properly logged.

A vessel operating on a Great Lakes route, in cold water, greater than 3 miles from shore must carry and inflatable liferaft with SOLAS B Pack with a capacity for 100% of the crew. On the date of inspection the liferaft was out being serviced and not on board. Provide a SOLAS B Pack liferaft with a capacity for 100% of the crew onboard the vessel and certificate of service for the same.

All bilges and voids spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. The aft space which contains the vessels steering pumps has 12-18’ of oily water mixture. Remove all water mixture.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Electrical cable used to supply power to tank barges has exposed wires on end of cable. Remove cable or install plug on end of cable.

A towing vessel that is certificated for service beyond 3 miles from shore on the Great Lakes, whose contract for construction was executed prior to August 27, 2003 must be equipped with either: (1) an approved 160-B semi-portable fire-extinguishing system to protect the engine room; or a fixed fire extinguishing system (meeting the definition in 46 CFR 136.110) installed to protect the engine room. The vessel is provided with a carbon dioxide system that has not been verified to meet the definition of fixed fire extinguishing system in 46 CFR 136.110. Provide verification the carbon dioxide system meets 46 CFR 136.110 or provide a B-V extinguisher for the engine room.

Conduct man-overboard and fire fighting drills with the full crew complement, in the presence of a Marine Inspector, prior to engaging in pushing, pulling, or hauling alongside.

The nontank VRP Interim Operating Authorization expired 7/14/07. The owner/operator has failed to verify the status of the vessel and/or recertify that response resources are capable of responding to the maximum extent practicable, to a worst case discharge. In addition, the VRP has inconsistencies in qualified individuals.

Repair and prove proper operation of engine room fire detection system IAW 46 CFR 27.203.

No remote start for firepump outside engine room as required under 46CFR27.301
