25NOV2007: Vessels reported port anchor is missing. Solas 2004 CH1 Reg 11/c. Vessel is to have a tug assist while entering and departing the port of Houston/Galveston. Vessel is to remain in constant communication with tug while in transit. Prior to departure vessel is to provide class report to local OCMI stating that vessel remains in class.

Mixed Garbage was found throughout the vessel even at the port side storage location before being discharged into the sea. The garbage log indicates that on 17 November the crew used their incinerator while in port (Balboa, Spain), which they deny. The electrician collects old batteries and make sure they are disposed of properly. However he is not listed in the garbage plan but should be.

During the lifeboat several of the crewmembers were directed to put on their immersion suits. They took over two minutes to get them on but they were never fully zipped up. This did not include the lifejacket, which is also required to be worn over the immersion suit. Recommended to the Master and [name withheld] more training needs to be done.

Resolution: Received a report from the Master advising us that crew training on how to put on immersion suits has been completed. He has assured us that continue training will be on going.

The crew failed the first fire drill, no boundary cooling was done, spaces adjacent to the one on fire were not inspected and the door leading to the fire was locked from inside. During the second fire drill one crewmember was not accounted for and the used of placing someone in the stretcher was questionable. More training needs to be done regarding fire fighting.
