Cargo ships shall carry one or more totally enclosed lifeboats comlying with reg. of section 4.6 of the Code of such aggregate capacity on each side of the ship as will accomodate the total number of persons on board. During the lowering of the port side lifeboat, as soon as the lifeboat reached the embarkation, the forward davit conenction released or broke causing the lifeboat to hang from the rear attachment only. Seconds later the rear attachment failed and lifeboat fell into the water. Before vessel can return to the U.S. It will be required to have one or more totally enclosed lifeboats of such aggregate capacity on each side of the ship. 74 SOLAS (20) III/ 31.1.1.1

Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship or the efficiency or completeness of its life-saving appliances... the master or owner of the ship shall report at the earliest opportunity to the Admin, the nominated surveyor or RO responsible for issuing the relevant certificate, who shall casue investigations to be initiated to determine whether a survey is necessary. Due to the accident that occured during PSC exam, USCG is recommending the nominated surveyor or RO who issued the SOLAS equipment certificate to conduct a survey of both lifeboats davits, wire ropes, and connection points on lifeboats. 74 SOLAS (20) I/ 11(c)
